Aflibercept (Zaltrap®) : une autorisation de mise sur le marché dans le cancer colorectal métastatique - 23/03/15

Résumé |
L'aflibercept (Zaltrap® : protéine de fusion recombinante, anti-angiogénique qui bloque l'activation des récepteurs du Vascular Endothelial Growth Factor [VEGF] et du Placental Growth Factor [PlGF]), est en attente d'une mise à disposition du produit en France. L'aflibercept a obtenu son autorisation de mise sur le marché (AMM) en Europe, le 2 février 2013 en association avec une chimiothérapie associant acide folinique, 5FU et irinotécan (FOLFIRI) dans l'indication : traitement des patients adultes atteints d'un cancer colorectal métastatique, résistant ou qui ont progressé après une chimiothérapie à base d'oxaliplatine. Cette décision a été fondée sur les résultats d'efficacités et de tolérance de l'étude VELOUR, qui a montré que l'association aflibercept avec acide folinique, 5FU, et irinotécan améliorait la survie globale vs la même chimiothérapie + placebo (13,5 mois vs 12,1 mois ; HR=0,817, IC à 95 %, 0,714 à 0,935 ; p=0,003 2). Les effets indésirables rapportés avec la combinaison d'aflibercept et de FOLFIRI incluaient les effets classiques des agents anti-angiogéniques et également une augmentation d'incidence de certaines toxicités en rapport avec la chimiothérapie. L'aflibercept est le deuxième agent anti-angiogénique à avoir une AMM dans le cancer colorectal métastatique.

Aflibercept (Zaltrap® : aflibercept is a recombinant fusion protein, anti-angiogenic, which blocks activation of VEGF and PIGF receptor), obtained its European approval February 2nd, 2013 in combination with Folinic Acid, 5FU and irinotecan (FOLFIRI) in adults with metastatic colorectal cancer that is resistant to or has progressed after an oxaliplatin containing regimen. This decision was based on the efficacy and safety results of the VELOUR Phase III trial. This study showed an improved overall survival, with the combination of aflibercept and FOLFIRI versus FOLFIRI + placebo (13.5 months vs 12.06 months ; HR=0,817, IC à 95 %, 0,714 à 0,935 ; p=0,003 2). Adverse effects reported with aflibercept combined with FOLFIRI included the specific anti–vascular endothelial growth factor effects and also reflected an increased incidence of some chemotherapy-related toxicities. Aflibercept is the second anti-angiogenic agent after bevacizumab with a FDA and a EMEA approval for patients with metastatic colorectal cancer.
