Cabazitaxel après docétaxel, une nouvelle option dans le cancer métastatique de la prostate résistant à la castration - 25/03/15

Résumé |
La prise en charge des patients avec un cancer de la prostate résistant à la castration est un vrai challenge. En effet jusqu'en 2011, après une première ligne de chimiothérapie par docétaxel, il n'existait aucun standard car les traitements étaient peu efficaces. Aujourd'hui, l'arsenal thérapeutique s'est étoffé avec l'arrivée de nouvelles thérapies. Parmi elles, le cabazitaxel, dérivé semi-synthétique des taxanes, a été développé pour sa faible reconnaissance par le système MDR et son pouvoir de diffusion notamment encéphalique. Cette nouvelle chimiothérapie a été évaluée en deuxième ligne chez des patients ayant progressé sous ou après chimiothérapie par docétaxel dans un essai prospectif de phase III TROPIC. L'association cabazitaxel et prednisone a permis d'augmenter le taux de survie globale médiane de 2,4mois par rapport à un traitement par mitoxantrone. La principale toxicité a été hématologique avec des risques d'aplasie fébrile et il est conseillé d'associer à ce traitement des facteurs de croissance hématopoïétiques en cas de neutropénies significatives. Le cabazitaxel, Jevtana®, a ainsi obtenu une autorisation de mise sur le marché (AMM) en deuxième ligne après docétaxel. Son positionnement par rapport aux hormonothérapies de nouvelle génération, comme l'acétate d'abiratérone, dans la même indication nécessite des investigations complémentaires, notamment l'identification de facteurs prédictifs de réponse. Des études sont en cours en première ligne de traitement (par rapport au docétaxel) et en association avec les nouvelles hormonothérapies.

The management of patients with metastatic castration-resistant prostate cancer is a real challenge. Indeed, after a first line chemotherapy with docetaxel, there was no standard because the treatments were ineffective. Today, several therapeutic options are available with the development of new therapies. Among them, cabazitaxel, semi-synthetic derivative of a natural taxoid, has been developed to its low recognition by the MDR system and power distribution including brain. This new chemotherapy was assessed in patients with metastatic castration-resistant prostate cancer whose disease has progressed during or after docetaxel-based therapy. Treatment with cabazitaxel plus prednisone has improved overall survival of 2.4months compared to mitoxantrone in the TROPIC phase III. However, hematologic toxicity may be limiting with a risk of febrile neutropenia; hematopoietic growth factors are advised in case of significant neutropenia. The cabazitaxel, Jevtana®, has been approved in second line after docetaxel. Its position in relation to new types of hormone therapy, as abiraterone acetate, in the same indication requires further investigations, including predictive factors of response. Studies are on going in first line indication (compared to docetaxel) and associated to other new hormone therapies.
