Background |
Trastuzumab with 5-fluorouracil (5-FU) and cisplatin offers prolonged survival in patients with HER2-overexpressing advanced gastric cancer (AGC) and advanced gastro-oesophageal junction cancer (AGOJ). Oxaliplatin in combination with intravenous 5-FU plus leucovorin (LV; modified [m]FOLFOX6) or capecitabine (XELOX) improves tolerability compared with 5-FU/cisplatin regimen. There are few data available on the efficacy and safety of trastuzumab-oxaliplatin-based chemotherapy in previously untreated HER2-positive AGC and AGOJ patients.
Methods |
Clinical data were retrospectively analysed in patients receiving trastuzumab plus mFOLFOX6 or XELOX as first-line therapy between July 2009 and December 2012. Eligible patients had histologically proven AGC or AGOJ, HER2 overexpression, and no prior chemotherapy for metastatic disease.
Results |
Thirty-four patients met the eligibility criteria. Median age was 63 years, 79% of patients had ECOG PS score of 0–1, and all had metastatic disease. Median duration of treatment was 7.5 months. Overall response rate was 41% (95% CI: 25–56). Median progression-free survival and overall survival were 9.0 months (95% CI: 5.6–12) and 17.3 months (95% CI: 13.5–32.3), respectively. Tolerability was acceptable. The most frequent grade 3–4 toxicities were neutropenia (8.8%) and neuropathy (17.6%).
Conclusion |
mFOLFOX6-trastuzumab combination is an efficient regimen with an acceptable safety profile for AGC and AGOJ patients. These results warrant further prospective study.

Contexte |
L’association trastuzumab-5-fluorouracile (5-FU)-cisplatine est le traitement recommandé aux patients avec adénocarcinome gastrique avancé (AGC) et d’adénocarcinome avancé de la jonction œso-gastrique (AGOJ) surexprimant HER2. L’oxaliplatine en combinaison avec le 5-FU intraveineux et l’acide folinique ([m]FOLFOX6 modifié) ou la capécitabine (XELOX) améliore la tolérance par rapport au 5-FU/cisplatine. Peu de données d’efficacité et de tolérance du trastuzumab-oxaliplatine sont disponibles chez ces patients avec AGC et AGOJ HER2-surexprimé non précédemment traités.
Méthodes |
Les données cliniques ont été analysées rétrospectivement chez des patients recevant du trastuzumab, associé à FOLFOX6m ou XELOX en traitement de 1re ligne entre juillet 2009 et décembre 2012. Les patients éligibles présentaient un AGC ou un AGOJ histologiquement prouvé, une surexpression de HER2 et n’avaient pas reçu d’autre traitement au stade métastatique.
Résultats |
Trente-quatre patients répondaient aux critères d’éligibilité. L’âge médian était de 63ans, 79 % des patients avaient un indice de performance ECOG 0–1, et tous avaient une maladie métastatique. La durée médiane du traitement était de 7,5 mois. Le taux de réponse global était de 41 % (IC 95 % : 25–56). La survie sans progression et la survie globale médiane étaient de 9,0 (IC 95 % : 5,6 à 12) et 17,3 mois (IC 95 % : 13,5 à 32,3), respectivement. La tolérance était acceptable. Les effets indésirables les plus fréquents de grade 3–4 étaient la neutropénie (8,8 %) et la neuropathie (17,6 %).
Conclusion |
Les associations FOLFOX6m ou XELOX-trastuzumab sont des régimes efficaces avec un profil de tolérance acceptable chez les patients avec AGC et AGOJ. Une étude prospective est souhaitable.
