Résumé |
HER2 (human epidermal growth factor receptor 2) est surexprimé dans 15 à 20 % des cancers du sein. L’avènement des thérapies ciblées anti-HER2, et notamment du trastuzumab, a transformé l’histoire naturelle de cette maladie. Le trastuzumab emtansine (T-DM1, Kadcyla®), composé du trastuzumab et de l’agent cytotoxique emtansine (ou DM1) couplés par un agent de liaison stable, a obtenu en novembre 2013 une Autorisation de mise sur le marché (AMM) européenne. Le trastuzumab emtansine cible et inhibe la voie de signalisation de HER2, tout en permettant de délivrer l’emtansine directement à l’intérieur des cellules cancéreuses HER2-positives. Il est indiqué en monothérapie dans le traitement de patientes prétraitées par taxane et trastuzumab pour un cancer du sein surexprimant HER2, d’évolution métastatique ou localement avancée non résécable, ou en rechute dans les 6 mois suivant un traitement adjuvant. Cette indication découle des résultats de l’étude EMILIA, essai ouvert de phase III randomisé contre la combinaison lapatinib-capécitabine. Le trastuzumab emtansine permettait une amélioration significative des deux critères de jugement principaux : la survie sans progression passait de 6,4 mois dans le bras lapatinib-capécitabine à 9,6 mois pour le bras trastuzumab emtansine (Hazard Ratio [HR]=0,65 ; intervalle de confiance à 95 % [IC 95 %]=0,55–0,77 ; p<0,001) ; et la survie globale de 25,1 mois à 30,9 mois (HR=0,68 ; IC 95 %=0,55–0,85 ; p<0,001) lors la deuxième analyse intermédiaire. Les taux d’évènements indésirables étaient supérieurs dans le bras lapatinib-capécitabine.

HER2 (human epidermal growth factor receptor 2) is overexpressed in 15 to 20% of breast cancer. Anti-HER2 targeted therapies, notably trastuzumab, have transformed the natural history of this disease. Trastuzumab emtansine, consisting of trastuzumab coupled to a cytotoxic agent, emtansine (DM1), by a stable linker, has been approved in November 2013 by the European Medicine Agency. Trastuzumab emtansine targets and inhibits HER2 signaling, but also allows emtansine to be directly delivered inside HER2-positive cancer cells. It is indicated as single-agent in taxane and trastuzumab-pretreated HER2-positive breast cancer patients with metastatic and locally recurrent unresecable disease or relapsing within 6 months of the end of adjuvant therapy. This indication is based on the results of the EMILIA study, an open label phase III randomized trial comparing trastuzumab emtansine to lapatinib-capecitabine. The two primary endpoints were reached. The progression-free survival was 6.4 months in the lapatinib-capecitabine arm versus 9.6 months for the trastuzumab emtansine arm (HR=0.65; 95% CI=0.55–0.77, P<0.001). Overall survival at the second interim analysis was 25.1 months in the lapatinib-capecitabine arm versus 30.9 months in the trastuzumab emtansine arm (HR=0.68; 95% CI=0.55–0.85, P<0.001). Moreover, adverse events were more frequent in the lapatinib-capecitabine arm.
