Nous rapportons le cas d’un patient atteint d’un syndrome d’attraction papillaire associé à une neurofibromatose de type II (NF2). Une enfant de 5 ans s’était présentée en consultation pour une amblyopie : meilleure acuité visuelle corrigée de 1/10e à droite et de 5/10e à gauche. L’examen retrouvait un syndrome d’attraction papillaire associé à une cataracte sous-capsulaire postérieure de l’œil droit. À gauche, il existait une membrane épirétinienne blanchâtre recouvrant la fovéa. Cinq ans plus tard, elle développait une exophtalmie gauche associée à une masse cervicale. L’IRM révélait un schwannome développé à partir des gaines du nerf optique à gauche associé à un schwannome cervico-thoracique. Le diagnostic de neurofibromatose de type 2 fut posé. L’association d’une cataracte sous-capsulaire postérieure et d’une membrane épirétinienne maculaire est très hautement suggestive de NF2. Le syndrome d’attraction papillaire n’a pas encore été décrit dans la NF2. Il peut s’agir d’une des manifestations de la maladie.
Dragged disc syndrome in a patient presenting neurofibromatosis type II: a case study |
We report a case of dragged disc syndrome associated with neurofibromatosis type II (NF2). A 5-year-old girl with amblyopia (right eye, 20/200; left eye, 20/40) presented with dragged disk syndrome and a posterior subcapsular cataract in the left eye associated with an epiretinal membrane in the right eye. Five years later, she developed exophthalmos in the right eye associated with a cervical mass. MRI revealed schwannoma developing in the left optic nerve sheath associated with a cervicothoracic schwannoma. The child was diagnosed with NF2. Association of posterior subcapsular cataract with macular epiretinal membranes is highly suggestive of NF2, although the dragged disk syndrome has not been previously reported in NF2 to our knowledge. It may also be an ocular feature of this disease.
