Voir les affiliationsMalgré des progrès constants dans l'assistance médicale à la procréation, le manque de connaissances et de contrôle du processus implantatoire demeure un important facteur d'échec de grossesse. Il est important de chercher à déterminer les caractéristiques d'un endomètre réceptif.
L'implantation embryonnaire est un processus complexe au cours duquel l'embryon va d'abord s'apposer, puis adhérer à l'endomètre maternel pour ensuite y pénétrer.
Alors que l'implantation peut se faire dans n'importe quel tissu de l'organisme, l'endomètre n'est favorable à l'implantation embryonnaire que pendant une période limitée appelée « fenêtre implantatoire ».
L'existence d'un dialogue materno-foetal est indispensable pendant cette période. Il reste encore à identifier les signaux prédominants de ce dialogue et à définir les caractéristiques d'un endomètre réceptif.
Dans ce cadre, le Professeur V. Geenen et le Docteur S. Perrier d'Hauterive, du Centre d'Immunologie de Liège, explorent le rôle de l'hormone chorionique gonadotrope (hCG), du Transforming Growth Factor ß1 (TGFß1), des insuline-like growth factors 1 et 2 (IGF1 et IGF2) dans la régulation de la sécrétion du LIF (Leukemia Inhibitory Factor) et d'IL6 (interleukine 6) par l'épithélium endométrial humain en culture.
Des cultures primaires de cellules épithéliales d'endomètre humain ont été réalisées à partir de biopsies d'endomètre réalisées chez 28 patientes fertiles.
Les cellules épithéliales ont été mises en culture pendant 72 h et elles ont été exposées à des facteurs embryonnaires exprimés précocement tels que l'IGF1, l'IGF2, l'hCG et le TGFß1.
hCG, TGFß1, IGF1 et IGF2 stimulent la production endométriale de LIF in vitro . Par ailleurs, l'hCG et le TGFß1 réduisent la production endométriale d'IL6 in vitro .
Ainsi, ces données suggèrent que des signaux précoces du blastocyste tels que hCG et TGFß1 joueraient un rôle actif dans l'implantation et dans la tolérance maternelle.

Despite progress in assisted reproduction technologies, the lack of control of implantation remains a major obstacle to obtain successful pregnancies. It is of prime importance to determine the characteristic features of a receptive endometrium. Embryo implantation is a complex event involving apposition followed by the adhesion of the blastocyst to the maternal endometrium, and finally the invasion of this endometrium. Though implantation could occur in any human tissue, the endometrium is the only tissue where embryo implantation cannot occur except during a restricted period called the “implantation window”. During this window, the endometrium is highly receptive to the embryo. Maternal-embryo crosstalk is favored by the implantation window. In the present study, we investigated the role of hCG, TGFß1, IGF1 and IGF2 in the secretion of leukemia inhibitory factor (LIF) and interleukin-6 (IL-6) by human epithelial endometrial cells in culture. Epithelial endometrial cells were obtained from endometrial biopsy in 28 fertile women.
HCG and TGFß1 added to primary cultures of human epithelial endometrial cells showed a stimulatory effect on LIF secretion and a reducing effect on IL-6 secretion.
All these findings suggest a role for hCG and TGFß1 in human embryo implantation.
