PRÉSERVATION DE L'AUDITION DANS LE TRAITEMENT PAR RADIOCHIRURGIE GAMMA KNIFE DES SCHWANNOMES VESTIBULAIRES UNILATÉRAUX - 10/03/08

Objectif de l'étude. — La majorité des patients opérés pour schwannomes vestibulaires perdent la fonctionnalité auditive malgré les progrès techniques microchirurgicaux. Nous avons souhaité évaluer les possibilités de préservation de la fonctionnalité auditive chez les patients opérés par Gamma Knife.
Matériel et méthodes. — Nous avons traité à Marseille, entre juillet 1993 et janvier 2002, 1 000 schwannomes vestibulaires. Une population de 175 patients traitée de première intention d'un schwannome unilatéral avec une audition préopératoire fonctionnelle (Gardner et Robertson 1 ou 2) a pu être étudiée avec un recul d'au moins 3 ans.
Résultats. — De nombreux paramètres influencent la probabilité de conservation auditive. Globalement, la probabilité de conserver une audition fonctionnelle est de 60 % à 3 ans. Les principaux paramètres de bon pronostic sont une perte auditive initiale faible, c'est-à-dire un grade 1 de Gardner et Robertson (versus 2), la présence d'un acouphène comme signe d'appel, le jeune âge du patient, la petite taille de la lésion. La probabilité de préserver une audition fonctionnelle est de 77,8 % chez les patients possédant initialement une audition subnormale (Gardner I), de 80 % chez les patients ayant comme premier symptôme un acouphène, et de 95 % chez les patients ayant les deux qualités. Chez ces même patients, la probabilité de conservation à 5 ans est de 84 %.
Conclusion. — Il s'agit d'une importante population de patients traités par radiochirurgie avec une audition fonctionnelle initiale. Les résultats obtenus permettent d'affirmer la possibilité de préserver une audition efficace dans un pourcentage très élevé de patients sélectionnés, donnant à ces patients une probabilité de conservation plus importante qu'avec la microchirurgie, et plus importante certainement qu'avec un simple suivi radiologique.
Preserving hearing function after Gamma Knife radiosurgery for unilateral vestibular schwannoma |
Objectif. — The majority of the patients still lose hearing function in spite of the technical advances in microsurgery. We wanted to evaluate preservation of hearing function potential after Gamma Knife Surgery.
Material and method. — In Marseille, we performed 1000 Gamma-Knife procedures for vestibular schwannomas between July 1992 and January 2002. This population included 175 patients undergoing first intention surgery for a unilateral schwannoma with functional preoperative hearing (Gardner and Robertson 1 or 2) who were studied with a follow-up longer than 3 years. Univariate and multivariate analysis was performed.
Results. — Numerous parameters significantly influenced the probability of functional hearing preservation at 3 years. The overall rate of preservation was 60%. The main parameters of predictability were limited hearing loss (Gardner and Robertson stage 1 versus 2) before radiosurgery, presence of tinnitus as the initial symptom, young age and the small tumor size. Preservation of functional hearing at 3 years was 77.8% when the patient was initially in stage I, 80% when the patient's first symptom was tinnitus, and 95% when the patient had both. In these patients, the probability of functional preservation at 5 years was 84%.
Conclusion. We report a large population of patients treated by radiosurgery with initial functional hearing. These results demonstrate that a large percentage of selected patients can preserve functional hearing after Gamma Knife Surgery, their chances of functional preservation being greater than after microsurgery or simple surveillance.
