Les syndromes paranéoplasiques sont des manifestations systémiques associées à certains cancers, ni liées à l’envahissement tumoral local ou métastatique ni expliquées par une cause carentielle, métabolique, infectieuse ou iatrogène. La présence d’anticorps peut aider au diagnostic de syndrome paranéoplasique, même si elle n’est pas indispensable. Les atteintes choroïdo-rétiniennes regroupent le syndrome Cancer Associated Retinopathy (CAR), le syndrome Melanoma Associated Retinopathy (MAR), la Maculopathie Vitelliforme Paranéoplasique et la Prolifération Bilatérale Diffuse des Mélanocytes Uvéaux. Les syndromes paranéoplasiques neurologiques associés à une atteinte ophtalmologique, comprennent la neuropathie optique, l’opsoclonus-myoclonus, le syndrome myasthénique de Lambert-Eaton, la dégénérescence cérébelleuse paranéoplasique (DCP) et la rhombencéphalite.

Paraneoplastic syndromes involving the visual system are a heterogeneous group of disorders occurring in the setting of systemic malignancy. Although these syndromes are rare, the clinical manifestations can herald an unsuspected, underlying malignancy. The associated antibodies and histopathology of each syndrome are presented to help in the understanding of these autoimmune phenomena. Outlined in this review article are diagnostic features useful in differentiating cancer-associated retinopathy, melanoma-associated retinopathy, paraneoplastic polymorphous vitelliform maculopathy, bilateral diffuse uveal melanocytic proliferation and paraneoplastic neurologic syndromes such as optic neuropathy, opsoclonus-myoclonus, Lambert-Eaton myasthenia and paraneoplastic cerebellar degeneration.
