Summary

Background

A two-step strategy of invasive management without stenting, guided by optical coherence tomography (OCT), in selected patients with acute coronary syndrome (ACS), might avoid systematic stent implantation and allow medical therapy alone.

Aims

To assess the feasibility and safety of such a procedure, and to define coronary imaging characteristics in a specific population.

Methods

This single-centre proof-of-concept study included all patients with ACS who benefited from a two-step revascularization procedure with optimal reperfusion during primary percutaneous coronary intervention followed by delayed angiography and OCT. OCT imaging determined medical therapy treatment alone without stenting in case of absence of vulnerable plaque rupture and <70% stenosis. Follow-up consisted of screening for major adverse cardiac events (MACE) at 12months.

Results

Forty-six patients were included, mainly men (86.9%) and smokers (65.2%), with a mean age of 47.1years. Most cases (80.4%) were large thrombus burden lesions. Delayed angiography and OCT were performed in a median period of 6 [3–10] days. No adverse events occurred between the initial and second angiograms. Plaque rupture was detected in 39.1% of patients, plaque erosion in 54.3% and calcified nodule in 6.5%. Twenty-three patients benefited from systematic delayed OCT over a median period of 171days, showing an increase in minimal lumen area. At 12months, two patients (4.3%) presented MACE and were stented. No sudden death or myocardial infarction recurrence occurred.

Conclusions

Analysing ACS mechanisms by OCT might facilitate treatment decisions in patients with ST-segment elevation myocardial infarction managed by a two-step procedure. Conservative treatment with antithrombotic therapy without stenting seems to be a reliable option in a selected population.

Résumé

Introduction

Une stratégie en 2 temps, guidée par l’imagerie par cohérence optique (OCT) dans la prise en charge du syndrome coronarien aigu (SCA) de patients sélectionnés, peut éviter la mise en place systématique d’un stent en privilégiant un traitement médical seul.

Objectif

Évaluer la faisabilité et la sécurité de cette stratégie et définir les caractéristiques morphologiques en imagerie endocoronaire validant cette stratégie.

Matériel et méthodes

Étude mono-centrique incluant tous les patients présentant un SCA ayant bénéficié d’une prise en charge en 2 temps : reperfusion optimale en phase aiguë suivie d’un contrôle coronarographique retardé avec OCT. L’OCT a permis la poursuite d’un traitement médical sans mise en place de stent en cas d’absence de rupture de plaque vulnérable et en l’absence de sténose de plus de 70 %. Un suivi médical de 12 mois était préconisé.

Résultats

Cent quatre-vingt-un patients ont été inclus, 46 ont bénéficié d’un traitement médical sans mise en place de stent, essentiellement des hommes (86,9 %), fumeurs (65,2 %) avec un âge médian de 47,1. Au total, 80,4 % des patients se présentaient avec une charge thrombotique importante lors de la procédure initiale. La procédure différée et l’OCT ont été réalisés après une période médiane de 6 jours (3–10). Aucun événement n’est survenu entre les 2 procédures. Une rupture de plaque a été retrouvée chez 39,1 % des patients, une érosion chez 54,3 %, un nodule calcifié chez 6,5 %. Vingt-trois patients ont bénéficié d’un contrôle tardif en OCT après une période médiane de 171jours montrant une augmentation de la surface de lumière artérielle. À 12 mois, 2 patients (4,3 %) ont présenté un évènement ischémique et ont été stentés. Il n’y a eu aucun infarctus ni décès.

Conclusion

L’analyse en OCT des mécanismes du SCA permet d’adapter la thérapeutique chez les patients présentant un SCA avec prise en charge en 2 temps. Un traitement médical seul sans mise en place systématique d’un stent semble être une option possible dans une population sélectionnée.
