Embolization of primary orbital varix via retrogradetransvenous approach

Background. Primary orbital varix isa congenital venous malformation. Active treatment include surgicalexcision, electrothrombosis, injection of sclerosing agents andembolization. Active treatment has only two indications: pain withhigh orbital pressure and esthetic disfigurement.

Methods. 3 cases of orbital varix were treated by coilsembolization via retrograde transvenous route. In all casespatients were symptomatic with pain, positional proptosis. CT scanshowed a large vascular retro bulbar mass. Via transfemoralvein approach a microcatheter was introduced via the homolateralinferior petrosal sinus into the cavernous sinus, the superior orinferior ophtalmic vein and into the orbital varix. Occlusion ofthe varix was performed with GDC coils (2 cases) and Rapid Detachcoils (1 case). After embolization the symptomatology disappearedimmediatly in 2 cases. The last patient developed slight conjonctivalinfection during 1 month.

Conclusion. Endovascular embolization of orbital varixvia transvenous retrograde route is an effective therapeutic beforesurgery who may lead to damage intraorbital nerves, muscles or vessels.
