What’s new in dermatological treatment?

La révolution thérapeutique dans la prise en charge des dermatoses inflammatoires est en marche. L’arsenal thérapeutique du psoriasis s’étoffe avec en plus de l’élargissement du champ des biologiques (anti-TNF, anti-IL12/IL23, anti-IL17, anti-IL23), un regain d’intérêt pour les petites molécules (anti-tyrosine kinase) et l’arrivée d’un nouvel acteur pour le psoriasis pustuleux généralisé (antirécepteur de l’IL36). Pour la dermatite atopique, la mise à disposition prochaine de nouveaux biologiques, en plus de l’anti-IL4/IL13, se précise avec l’arrivée de traitements spécifiques du prurit (biologique et voie orale). Les anti-JAK confirment leur intérêt dans la pelade avec une première étude randomisée contre placebo. Ces molécules apparaissent aussi intéressantes dans la dermatomyosite. Une autre révolution technologique est marquée par de nouveaux agents thérapeutiques : les petits ARN interférents qui font une entrée tonitruante dans la prise en charge d’une maladie orpheline, l’amylose héréditaire.

The therapeutic revolution in the management of inflammatory dermatoses is under way. The therapeutic arsenal is expanding in the field of psoriasis, including biologics (TNF blockers, anti-IL12/IL23, anti-IL17, and anti-IL23 antibodies), new small molecules (tyrosine kinase inhibitor), and a new biologic for generalized pustular psoriasis (anti-IL36 receptor). New biologics will be soon available in the field of atopic dermatitis in addition to anti-IL4/IL13 antibodies. New targeted treatments of pruritus are also coming (biologics and small molecules). A first randomized placebo-controlled trial has confirmed the interest of JAK inhibitors in alopecia areata. These molecules seem to be also promising in dermatomyositis. Another therapeutic revolution will be technological with the development of new therapeutic agents: small interfering RNA. Recent clinical trials confirmed their efficacy in hereditary amyloidosis.
