Introduction |
Nous rapportons un cas d'impétigo streptococcique survenu chez une jeune fille traitée par tacrolimus en pommade (Protopic®) pour une dermatite atopique.
Observation |
Une jeune fille de 15 ans était traitée par tacrolimus 0,03 p. 100 (Protopic®) pour une poussée de dermatite atopique. À la décroissance des applications du tacrolimus, survenait une éruption vésiculo-pustuleuse traitée par pristinamycine et valaciclovir. À la fin du traitement antibiotique et antiviral, une récidive de l'éruption vésiculo-pustuleuse survenait alors que la malade était de nouveau traitée par tacrolimus pommade à 0,1 p. 100. Les prélèvements bactériologiques et virologiques cutanés trouvaient un streptocoque β hémolytique du groupe A. La négativité des prélèvements virologiques cutanés éliminait un syndrome de Kaposi-Juliusberg. Le diagnostic d'impétigo streptococcique était posé. L'imputabilité intrinsèque du tacrolimus était I3 (C3 S2).
Discussion |
La particularité de cet impétigo était sa localisation exclusive sur les zones d'eczéma traitées par l'application de tacrolimus. Dans les études prospectives sur de grandes cohortes, le tacrolimus en pommade provoque deux types d'effets secondaires : d'une part des irritations locales, d'autre part des infections cutanées majoritairement causées par le Staphylococcus aureus. L'impétigo à Streptococcus béta hémolytique, après application de tacrolimus n'a jamais été relaté dans la littérature. Le tacrolimus en pommade, par son effet immunodépresseur devrait entraîner une incidence accrue des infections cutanées même si plusieurs études montrent plutôt une diminution de l'incidence des infections cutanées.

Background |
We report a case of staphylococcal impetigo in a girl treated with tacrolimus ointment (Protopic®) for atopic dermatitis.
Observation |
A 15 year-old girl was receiving treatment with tacrolimus 0.03% (Protopic®) for an episode of atopic dermatitis. On reduction of applications of tacrolimus, a vesicular-pustular rash appeared and was treated with pristinamycin and valaciclovir. At the end of antibiotic and antiviral therapy, the vesicular-pustular rash recurred while the goal was receiving treatment once more with tacrolimus ointment 0.1%. The bacteriological and virological skin samples revealed β-haemolytic streptococcus group A. The negative results for cutaneous virological samples ruled out Kaposi-Juliusberg syndrome and a diagnosis of staphylococcal impetigo was made. The intrinsic imputability of tacrolimus was I3 (C3 S2).
Discussion |
The most obvious specific feature of this impetigo was its limitation to areas of eczema treated by application of tacrolimus. In prospective studies in large patient cohorts, tacrolimus ointment has been associated with two types of adverse effect: local irritations and skin infections chiefly caused by Staphylococcus aureus. To date, there have been no reports in the literature of impetigo due to haemolytic β streptococcus following application of tacrolimus. Because of its immunodepressant effect, tacrolimus ointment may result in increased incidence of skin infections even though a number of studies have shown a reduction in such infections.
