Introduction

Les métastases de la cavité buccale sont rares. Nous rapportons l'observation d'une métastase gingivale au cours d'un sarcome phyllode du sein habituellement peu métastatique.

Observation

Une femme, âgée de 44 ans, était opérée 2 ans auparavant pour un sarcome phyllode du sein confirmé histologiquement. Elle consultait pour une tumeur exophytique mandibulaire associée à une gingivite ulcéro-bourgeonnante, hémorragique et nécrotique des gencives mandibulaire et maxillaire. L'examen clinique trouvait des lésions nodulaires multiples du tronc et du cuir chevelu. Les examens histologiques confirmaient le diagnostic de métastases gingivale et cutanée d'un sarcome phyllode du sein. L'évolution était rapidement fatale.

Discussion

Les métastases gingivales sont des tumeurs rares de mauvais pronostic. Leur diagnostic se pose sur des arguments cliniques et histologiques et doivent être différenciées des gingivites chroniques souvent associées.

Gingival metastasis in breast phyllode sarcoma.
Background

The metastasis of oral cavity is rare. We report a gingival metastasis in the course of low grade phyllode sarcoma of the breast which is usually little metastatic.

Case-report

A 44 year old woman was operated two years ago for a phyllod sarcoma of the breast histologically confirmed. She presented for a mandibular exophytic tumour associated with haemorrhagic, necrotic, ulcerated and bulking gingivitis surrounding at the level of the maxillary and mandibular gingiva. The examination revealed multiple nodular lesions of the trunk and the scalp. The histological examination confirmed the diagnosis of cutaneous and gingival metastasis of the phyllod sarcoma of the breast. The evolution was rapidly fatal.

Discussion

The gingival metastasis are rare tumours with a bad prognostic. Their diagnosis is based on clinical and histological arguments and must be made different from chronic gingivitis which can be associated.
