Résumé |
Nous rapportons l'observation d'une famille affectée d'une forme atypique d'ostéo-arthropathie hypertrophiante primitive, forme caractérisée par une atteinte limitée aux membres inférieurs et ne comportant ni hippocratisme digital, ni modifications cutanées. Trois membres de cette famille étaient atteints de douleurs avec gonflement et hypersudation des deux pieds. Leurs radiographies montraient des modifications à la fois de résorption et de formation osseuses des métatarsiens avec prolifération périostée près de l'astragale. La scintigraphie osseuse au technétium mettait en évidence une hyperfixation précoce. La biopsie osseuse et synoviale montrait une fibrose médullaire avec augmentation de la résorption ostéoclastique et la présence d'une synoviale de résorption. Nous avons exclu toute autre pathologie ostéo-articulaire de type infectieux, neurologique, métabolique ou néoplasique.

Abstract |
We report a case of a family with a variant form of primary hypertrophic osteoarthropathy restricted to the lower extremities without digital clubbing or cutaneous changes. Three family members suffered of pain, swelling and hyperhidrosis of both feet. X-rays showed destruction and osteoproliferative changes of the metatarsal bones with periostal hyperostosis close to the talus. There was an increased 99m-Tc-MDP uptake in the early phase of bone scintigraphy. Fibrosis of the marrow with stimulated osteoclastic resorption and the presence of detritus synovialitis were visible in a bone and joint biopsy. All known infectious, neurologic, metabolic and malignant diseases, which affect the bone and joints, were excluded.
