Introduction |
Au cours du lupus, l'anticoagulant circulant acquis le plus fréquent est l'anti-prothrombinase ; il est responsable de thromboses. La présence d'anticorps anti-facteur VIII est rarement observée au cours du lupus érythémateux systémique. Nous rapportons une observation d'hémophilie acquise chez une malade lupique.
Observation |
Une femme âgée de 30 ans, suivie pour lupus érythémateux systémique, consultait pour une coxalgie droite et des lésions ecchymotiques prédominantes au membre supérieur droit. Les examens complémentaires mettaient en évidence des facteurs antinucléaires au 1/2 560, des anticorps anti-DNA à 300 UI/ml, une anémie, un allongement du temps de céphaline activée, et surtout un facteur VIII à 1 p. 100 et la présence d'un anticorps anti-facteur VIII. Une imagerie par résonance magnétique de la hanche droite confirmait l'existence d'une collection hématique intramusculaire. La malade était initialement traitée par des corticoïdes en bolus et per os, des perfusions d'immunoglobulines et des bolus de cyclophosphamide. L'évolution était favorable tant sur le plan clinique que biologique.
Discussion |
La symptomatologie hémorragique chez cette malade lupique a été révélatrice d'une hémophilie acquise liée à la présence d'anticorps anti-facteur VIII. La rareté des observations d'hémophilie acquise au cours du lupus érythémateux systémique est à souligner et peut expliquer l'absence actuelle de consensus thérapeutique.

Introduction |
In patients with lupus, the most common acquired circulating anticoagulant is antiprothrombinase which is responsible for thrombosis. The presence of antibodies directed against factor VIII is rarely found in systemic lupus erythematosus. A case of acquired haemophilia in a patient with lupus is reported.
Case report |
A 30 year-old woman with systemic lupus erythematosus developed a right coxalgia and ecchymotic skin lesions which were prominent on the right arm and forearm. Laboratory values were as follows: positive antinuclear antibodies > 1: 2 560, anti-DNA antibodies (300 IU/ml), prolonged activated partial thromboplastin time, reduced factor VIII activity (1 p. 100) and the presence of antibodies against factor VIII. Magnetic nuclear resonance of the right hip confirmed the presence of an intramuscular hematoma. The patient was initially treated with intravenous pulse and oral corticosteroids, intravenous immunoglobulins and intravenous cyclophosphamide. Clinical and biological improvement was promptly obtained.
Discussion |
In our patient with systemic lupus erythematosus, bleeding revealed acquired haemophilia with antibodies against factor VIII. It should be pointed out that the association between lupus and haemophilia is uncommon and that at present no standardized treatment can be recommended.
