Cellulite faciale révélatrice d'une ostéomyélite mandibulaire chez un nourrisson - 29/04/08

Introduction |
La découverte d'une tuméfaction jugale de l'enfant peut faire évoquer plusieurs affections. Nous rapportons l'observation d'un nourrisson de 4 mois dont la cellulite de la joue a été révélatrice d'une ostéomyélite mandibulaire.
Observation |
Un garçon de quatre mois, était amené en consultation pour une tuméfaction jugale gauche, dure, chaude, avec de multiples adénopathies cervicales. Le diagnostic évoqué était une cellulite à point de départ cutané. Il existait une hyperleucocytose à 21 900/mm3 associée à un syndrome inflammatoire biologique. La radiographie standard du massif facial était normale. Une échographie de la face montrait un épaississement des tissus mous sous-cutanés et des adénopathies rétro et sous mandibulaires avec une image d'abcédation. Une antibiothérapie par amoxicilline et acide clavulanique permettait une rapide amélioration. Trois jours après l'arrêt de l'antibiothérapie, il existait une récidive de la tuméfaction sans fièvre. Un scanner de la face éliminait un lymphangiome kystique et montrait une ostéolyse de la table externe de la branche montante de la mandibule gauche avec appositions périostées. L'examen histologique d'une biopsie osseuse chirurgicale trouvait un aspect d'ostéite infectieuse et un streptocoque ß hémolytique était identifié en culture. L'évolution sous antibiothérapie (amoxicilline et gentamycine initialement puis amoxicilline en monothérapie) à 6 semaines était satisfaisante avec régression de tout signe cutané.

La découverte d'une tuméfaction dans cette localisation doit faire éliminer des causes tumorales malignes ou bénignes telles que le lymphangiome kystique. Les causes infectieuses (abcès, parotidite et ostéomyélite) doivent être évoquées et distinguées de l'hyperostose corticale infantile (syndrome de Caffey-Silverman). L'imagerie par radiologie standard, scanner ou scintigraphie est une aide au diagnostic. La découverte d'une ostéolyse doit conduire à un geste biopsique pour examen anatomopathologique et bactériologique.

Introduction |
The discovery of a jugular tumefaction in an infant evokes several diseases. We report the case of a 4-month-old infant whose jugular cellulite revealed mandibular osteomyelitis.
Case report |
A 4–month-old boy was referred for hard, hot tumefaction of the right cheek and multiple cervical adenopathies. The suggested diagnosis was cellulite of cutaneous origin. He presented 21 900/mm 3 hyperleukocytosis associated with an inflammatory biological syndrome. Standard x-ray of the facial mass was normal. Sonography of the face showed thickening of the soft subcutaneous tissues and retro and sub-mandibular adenopathies with abcedation. Antibiotherapy with amoxicillin and clavulanic acid led to rapid improvement. Three days after withdrawal of the antibiotherapy, the tumefaction recurred without fever. A facial scan eliminated cystic lymphangioma and showed osteolysis of the external plateau of the ascending branch of the mandible with periosteal appositions. Histological examination of a surgical bone biopsy showed infectious osteitis and culture revealed hemolytic ß streptococci. Six weeks of antibiotherapy (initially with amoxicillin and gentamycin, then amoxicillin in monotherapy) led to the regression of all cutaneous signs.
Comments |
When confronted with a tumefaction in this area, malignant or benign tumoral causes such as cystic lymphangioma must be eliminated. Infectious causes (abscess, parotid inflammation and osteomyelitis) must be evoked and distinguished from infantile cortical hyperostosis (Caffey-Silverman's syndrome). Standard radiological imaging, scan or scintigraphy are useful diagnostic tools. If osteolysis is discovered, a biopsy must be taken for anatomopathological and biological examination.
