Trouver une corrélation en terme de terrain auto-immun entre urticaire chronique et thyroïdite à l'aide du test au sérum autologue et de la recherche d'auto-anticorps antithyroïdiens.
Malades et méthode |
Étude dans un seul centre de 59 malades ayant consulté pour une urticaire évoluant depuis plus de 6 semaines sans étiologie précise. Réalisation d'un test au sérum autologue et recherche a posteriori d'auto-anticorps antithyroïdiens dans le sérum conservé après réalisation du test.
Résultats |
Sur 59 malades, 51 ont eu un test au sérum autologue interprétable et 57 ont eu une recherche d'auto-anticorps antithyroïdiens. Douze malades sur 57 (21 p. 100) avaient un ou plusieurs types d'autoanticorps antithyroïdiens positifs (n = 10) ou limites (n = 2), 8 sur 12 (66,6 p. 100) avaient une affection thyroïdienne connue avant le début l'urticaire. L'étude systématique sérique a permis de découvrir dans 4 cas des auto-anticorps antithyroïdiens méconnus. Deux de ces malades ont eu une rémission des symptômes sans traitement spécifique, une a été traitée par L thyroxine (hypothyroïdie) sans amélioration des symptômes et une autre malade était euthyroïdienne. Sur ces 12 sujets, 11 ont eu un test au sérum autologue, une positivité franche n'a été trouvée que pour 3 d'entre eux et un cas était douteux.
Conclusions |
Dans la limite de cette étude, nous trouvons un taux de 21 p. 100 de malades avec urticaire chronique ayant un ou plusieurs auto-anticorps antithyroïdiens. La positivité au test au sérum autologue ne semble pas être associée avec des anomalies auto-immunes thyroïdiennes.

Introduction |
The autoimmune nature of certain forms of chronic urticaria remains debatable. Aim of the study. To find a correlation in terms of autoimmune pattern between chronic urticaria and thyroiditis using the autologous serum test and the search for anti-thyroid antibodies.
Patients and methods |
Study in a single center of 59 patients having consulted for urticaria that had progressed for more than six weeks and without precise etiology. An autologous serum test was conducted and anti-thyroid autoantibodies were searched for in the serum stored after the test.
Results |
Out of 59 patients, 51 had an interpretable autologous serum test and 57 underwent research for anti-thyroid antibodies. Twelve patients out 57 (21 p.cent) exhibited one or more types of positive (n=10) or borderline (n=2) anti-thyroid antibodies, and eight out of 12 (66.6 p.cent) had a known thyroid disease before the onset of urticaria. The systematic blood test permitted the discovery of 4 cases of unknown anti-thyroid antibodies. The symptoms regressed in two of these patients without any specific treatment, one patient was treated with L thyroxin for hyperthyroidism without any improvement in the symptoms and one other patient had normal thyroid function. Among these 12 patients, 11 underwent autologous serum tests, but only 3 of them exhibited clearly positive results and one was doubtful.
Conclusions |
Within the limits of this study, we found a 21 p.cent rate of patients with chronic urticaria exhibiting one or more anti-thyroid antibodies. The positivity of the autologous serum test does not appear to be related with autoimmune thyroid disorders.
