Introduction |
La pachydermodactylie est une fibromatose superficielle située en regard des premières phalanges et des articulations interphalangiennes proximales des doigts. Divers types ont été décrits en fonction de la topographie, des facteurs étiologiques et des associations pathologiques. Nous rapportons une observation typique de pachydermodactylie associée à une pachydermie plantaire.
Observation |
Un jeune homme, âgé de 19 ans, était suivi pour une dysharmonie psychotique se manifestant par un retard mental modéré. Il avait une pachydermodactylie typique prédominant sur le deuxième, troisième et quatrième doigt des deux mains, associée à une acrocyanose récente. Il existait en outre un aspect pachydermique sur la partie externe de la plante des pieds. L'examen histopathologique était identique sur les lésions digitales et les lésions plantaires avec une hypertrophie dense du collagène, envahissant les septums hypodermiques sur le pied.
Discussion |
Cette observation s'intègre dans la forme la plus classique de pachydermodactylie qui touche plusieurs doigts des deux mains, le plus souvent chez l'homme. On trouvait un contexte psychiatrique et des microtraumatismes répétés fréquemment signalés dans la littérature. Cependant, l'importance de la gêne fonctionnelle et l'acrocyanose étaient inhabituelles. L'association à une pachydermie plantaire n'a jusqu'alors jamais été signalée. L'aspect histologique identique sur les doigts et sur la plante des pieds a conduit à intégrer la pachydermodactylie dans un cadre plus vaste de pachydermie acrale.

Introduction |
Pachydermodactyly is a superficial fibromatosis located on the proximal portion of fingers'phalanges and interphalangeal joints. Several types of this disease have been described depending on topography, etiology and pathological associations. We report a typical observation of pachydermodactyly associated with a plantar pachydermy.
Case-report |
A 19 year-old man was followed for a psychotic disease, associated with mental retardation. On clinical examination, he showed a typical pachydermodactyly, predominantly located on the second, third and fourth fingers of both hands, associated with recent acrocyanosis. A pachydermic aspect was also observed on the external part of the feet sole. Histopathological analysis was identical on digital and plantar lesions with collagen swelling. These lesions extended into the subcutaneous fat only in the feet.
Discussion |
This observation is compatible with the most classical form of pachydermodactyly which involves several fingers of both hands, usually in men. Furthermore, the association with a psychiatric disorder and the notion of repeated traumatisms have been frequently reported in the literature. However, acrocyanosis and the important disability observed in our case are unusual. Association with plantar pachydermy has never been reported. The identical histological aspect on finger and feet sole suggests that pachydermatodactyly should be integrated in framework of acral pachydermy.
