Introduction |
Les antihistaminiques H1 (anti-H1) sont le traitement de référence de l'urticaire chronique. Nous rapportons cinq cas d'urticaire, induites ou aggravées par les antihistaminiques H1.
Méthodes |
Les explorations immunoallergologiques comprenant des prick-tests et intradermoréactions avec l'anti-H1 en cause ont été réalisées.
Résultats |
Les tests cutanés ont confirmé le caractère non-IgE dépendant des poussées urticariennes induites par les anti-H1 et un traitement par un antileucotriène (montélukast, Singulair®) a été efficace dans l'urticaire chronique chez 3/4 malades.
Discussion |
Deux hypothèses sont discutées pour expliquer l'effet aggravant paradoxal des anti-H1 sur l'urticaire : 1) les malades seraient sensibles à l'effet toxique, pro-inflammatoire du médicament, à l'origine d'une activation non spécifique du mastocyte ; 2) le fait que l'urticaire soit sensible aux antileucotriènes suggère que l'histamine n'est pas le médiateur principal de l'urticaire chez ces malades.

Background |
H1 antihistamines (anti-H1) are the treatment of choice in chronic urticaria. We report five cases of urticaria, induced or aggraved by H1 antihistamines.
Methods |
The immunoallergological investigations included prick-tests and intradermal tests with the antihistamine responsible for acute urticaria.
Results |
The skin tests confirmed the non-IgE dependent nature of the urticarial eruptions and anti-leukotrienes (montelukast, Singulair®) were effective in controlling chronic urticaria in 3/4 patients.
Discussion |
Two hypotheses are discussed to explain the paradoxical aggravating effect of H1 antihistamines on the urticaria: 1) the patients are sensitive to the toxic, pro-inflammatory effect of the drug, which is the source of nonspecific activation of mast cells; 2) the fact that the urticaria is sensitive to anti-leukotrienes suggests that histamine is not the principal mediator of urticaria in these patients.
