Objectif : Évaluerles performances de la cholangiographie par IRM (CIRM) dans la détectiondes complications biliaires après transplantation hépatiqueorthotopique.
Matériel et méthode : 27 patientsgreffés hépatiques suspects de complication biliaireont bénéficié de 34 cholangiographiespar IRM et directes. L’interprétation des CIRM étaitréalisée en aveugle des données cliniqueset biologiques par deux observateurs indépendants. L’interprétationdes lésions était effectuée segment parsegment après avoir divisé l’arbre biliaireen 7 segments anatomiques. Pour chaque segment, l’évaluationportait sur : la présence d’une dilatationbiliaire, l’existence de sténose, la présencede matériel intra-canalaire. Les résultats dela CIRM étaient comparés à ceux de lacholangiographie directe.
Résultats : 216 (98 %)des 221 segments évalués étaientanalysables en CIRM, la visualisation était bonne ou excellentepour 179 (80 %). L’analyse segment parsegment montrait que la sensibilité, spécificité,et fiabilité globale de la CIRM était respectivementde 85 %, 81 % et 83 % pourdétecter une sténose biliaire, de 82 %,81 % et 81 % pour détecter unedilatation biliaire, et était de 60 %,88 % et 80 % pour détecterla présence de matériel intra-canalaire.
Conclusion : Lacholangiographie-MR est performante pour détecter des sténoseset des dilatations des voies biliaires chez le greffé hépatique etoffre une alternative aux opacifications directes.
Purpose: To assess the value of MRCPin the detection of biliary complications after orthotopic livertransplantation.
Materials and methods: 27 transplantedpatients with suspected biliary complication underwent a total of34 MR and direct cholangiography procedures. MRCP were reviewedby 2 independent reviewers blinded to clinical and laboratory findings.The biliary tract was divided into 7 segments, and alllesions were evaluated using this segmental anatomy. Each segmentwas evaluated for the presence of dilatation, stenosis and intra-ductaldebris. MRCP results were compared to results frpm direct cholangiography.
Results: 216(98%) of 221 biliary segments could be evaluated on MRCP,with good to excellent visualization in 179 (80%) cases. Segmentalanalysis showed sensitivity, specificity and accuracy values of85%, 81% and 83% for the detection ofbiliary stenosis, 82%, 81% and 81% forthe detection of biliary dilatation, and 60%, 88% and80% for the detection of inyraductal debris.
Conclusion: MRCPis accurate for the detection of biliary stenosis and dilatationin patients after liver transplantation and provides an alternativeto direct cholangiography.
