Nous décrivons ici l’apport de la vidéo-endoscopie bronchique à visée diagnostique en pédiatrie. Cette technique, très performante, est utilisable dès la période néonatale. Ses indications sont différentes selon l’âge de l’enfant. Chez le nouveau-né, elle a sa place dans le diagnostic d’une malformation des voies aériennes, le bilan d’un stridor et la localisation pré-opératoire d’une fistule trachéo-œsophagienne. Chez l’enfant plus grand, ces indications sont élargies à la pathologie bronchique aiguë ou chronique. En réanimation, cet examen est incon-tournable dans le suivi d’un ventilé chronique (trachéotomie), dans l’exploration d’une hémoptysie ou d’un trouble de ventilation et dans le diagnostic d’un stridor persistant après extubation. L’apport de la vision directe et la possibilité d’enregistrement numérique font de cette exploration un outil diagnostique et d’enseignement de qualité.

The author describes the use of paediatric flexible videobronchoscopy. Examinations were performed in newborn, infant and child. In neonatal period, the technique is used for the diagnostic of congenital malformation of airway, upper airway obstruction, pre operative evaluation of tracheooesophageal fistula. In children, more frequent indications are airway obstruction with non clear cause, stridor, chronic cough, congenital malformation of airway, tracheomalacia, follow-up of lung transplantation. In intensive care unit, this technique is very useful in the cases of atelectasis, stridor post extubation, follow-up in prolonged intubation and tracheotomy. The possibility of direct visualization of the anomalies in screen and the function of numeric record are important for diagnosis and teaching procedure.
