Résumé

Les manifestations digestives de la maladie de Kawasaki sont généralement limitées à une stomatite, des diarrhées, un iléus paralytique ou un hydrocholécyste. La colite ischémique et le syndrome d’hémophagocytose sont des complications rarement rapportées.

Observation. – Une fille âgée de cinq ans a été hospitalisée pour une maladie de Kawasaki compliquée d’un syndrome d’hémophagocytose d’évolution favorable après traitement par immunoglobulines polyvalentes. Au quatrième jour d’hospitalisation, elle a présenté des douleurs abdominales et une diarrhée glairosanglante aseptique. La tomodensitométrie abdominale a montré un aspect de colite ischémique touchant tout le côlon et l’intestin grêle. Les manifestations digestives se sont améliorées après repos digestif et alimentation parentérale totale.

Conclusion. – La colite ischémique et le syndrome d’hémophagocytose sont des complications rares mais parfois graves de la maladie de Kawasaki.

Abstract

Gastrointestinal manifestations of Kawasaki disease are usually limited to stomatitis, paralytic ileus, and hydrops of the gallbladder. We report a case of Kawasaki disease complicated with hemophagocytosis and ischemic colitis.

Case report. – A 5-year-old girl with Kawasaki disease presented with hemophagocytosis that responded to gamma-globulin therapy. On day 4 she had abdominal pain and diarrhea. CT scan showed features suggesting ischemic colitis. Symptoms resolved on total parenteral nutrition.

Conclusion. – Ischemic colitis and hemophagocytosis are potential severe complications of Kawasaki disease.
