Abstract |
Subperiotal hematoma of the orbit associated with extradural hematoma has been rarely reported in the literature. We report a case of an 8-year-old girl who presented after traumatism, a subperiotal hematoma of the orbit with two extradural hematomas. During the first clinical examination realized while the patient was unconscious, left exophthalmia was found, but after conscious improvement, we discovered ophtalmoplegia and a grade I papilla edema. The CT scan showed two extradural hematomas and a left intraorbital hematoma. The intraorbital hematoma was drained by surgical way but the extradural hematomas were not surgical. The outcome has been marked by the neurological improvement and the oculomotricity recovering.

L’association d’un hématome sous-périosté orbitaire à un hématome extradural (HED) a été rarement décrite dans la littérature. Nous rapportons l’observation d’une fillette de huit ans qui a présenté, à la suite d’un polytraumatisme, un hématome sous-périosté orbitaire, associé à deux lames d’hématomes extraduraux. L’examen initial, réalisé pendant que la patiente était inconsciente, n’a objectivé qu’une exophtalmie gauche, mais la réévaluation ophtalmologique, effectuée après l’amélioration de l’état neurologique, a retrouvé une ophtalmoplégie et un œdème papillaire stade I. Le scanner crânio-orbitaire a objectivé deux lames d’hématomes extraduraux pariétal et temporal gauches et un hématome intraorbitaire homolatéral. L’hématome intraorbitaire a été évacué par incision sourcilière, alors que les lames d’hématomes extraduraux ne relevaient pas d’un traitement chirurgical. L’évolution a été marquée par une amélioration neurologique et une récupération de l’oculomotricité.
