Thromboses multiples associées à deux tumeurs occultes : une observation simulant un syndrome catastrophique des antiphospholipides

Résumé

Des phénomènes thromboemboliques sont souvent associés au cours de l’évolution naturelle des tumeurs. Cependant, il est assez rare que la manifestation inaugurale d’une tumeur soit des thromboses multiples simulant un syndrome catastrophique des antiphospholipides. Nous décrivons ici l’observation d’une patiente âgée de 51ans qui a présenté de multiples thromboses cérébrale, pulmonaire et rénale. Malgré l’administration de bolus de glucocorticoïdes, d’un traitement anticoagulant et de perfusion d’immunoglobulines, la patiente est décédée. Elle souffrait de deux tumeurs occultes qui n’ont pas pu être identifiées en prémortem : un adénocarcinome pulmonaire et un cholangiocarcinome intrahépatique. Cette observation souligne l’importance de rechercher l’étiologie sous-jacente des thromboses multiples. Le facteur pronostique le plus important est l’instauration rapide du traitement antithrombotique et du traitement étiologique.
