Résumé |
La procédure du ganglion sentinelle (GS) est devenue en 15 ans le standard de prise en charge axillaire des carcinomes mammaires mesurant moins de 20mm sans ganglion palpable. De multiples sites d’injection ont été rapportés, comme l’injection péritumorale ou périaréolaire. Le traceur utilisé était un colorant bleu associé ou non avec un colloïde radioactif. L’injection péritumorale apporte des informations sur le drainage lymphatique profond et notamment celui de la chaîne mammaire interne. L’injection périaréolaire apporte une simplicité au geste, notamment en cas de tumeur infraclinique ou multifocale. Les résultats des essais multicentriques semblent montrer des taux d’identification légèrement supérieurs avec l’injection superficielle (périaréolaire ou péritumorale) avec une non-infériorité en termes de faux négatif (FN) en comparaison avec l’injection péritumorale profonde. Cependant, le taux de FN n’a jamais été formellement démontré pour l’injection périaréolaire exclusive.

Sentinel lymph node (SLN) mapping and biopsy have emerged as the technique of choice for axillary staging of breast cancer. Several methods have been developed to identify SLNs, including peritumoral or periareolar injection of blue dye or technetium colloid. The optimal site for injection of mapping tracers is controversial in SLN. The peritumoral injection provides information on the deep lymphatic drainage and the internal mammary chain. The advantages of periareolar injection are simplicity, the ability to perform it in non-palpable tumors, and the potential enhancement of uptake via the subareolar lymphatic plexus. The results of multiinstitutional study have indicated that superficial injection (periareolar or peritumoral) is associated with a better identification rate and an equal false-negative rate compared to deep peritumoral (PT) injection. However, the false-negative rate of periareolar injection has not been formally demonstrated.
