Example of a protocol for creating a direct oral anticoagulant therapeutic window for surgery or an invasive procedure with a low risk of bleeding (A) or a moderate or high risk of bleeding (B). In the case of bridging with a heparin (situation B), no overlap of the two anticoagulants is allowed, either preoperatively or postoperatively. Direct oral anticoagulant (DOA); low molecular-weight heparin (LMWH); unfractionated heparin (UFH); venous thromboembolism (VTE).
