Résumé |
Au cours d'une méningite sans germe, l'existence d'un syndrome inflammatoire oriente vers une cause bactérienne alors que la détection de l'interféron alpha (IFN-α) dans le liquide céphalorachidien plaide pour une méningite virale. La coexistence d'un syndrome inflammatoire et de la présence de l'IFN-α dans le liquide céphalorachidien rend cette distinction difficile. L'observation rapportée en fournit l'illustration et pose la question de la démarche diagnostique et de l'attitude thérapeutique requise.
Observation. - Un nourrisson âgé de six semaines, alimenté exclusivement au sein, a été hospitalisé en raison d'une fièvre. Les investigations révélaient un important syndrome inflammatoire et une atteinte méningée avec une cellularité à 94/mm3 dont 53 % de polynucléaires, contrastant avec une protéinorachie et une glycorachie normales. L'IFN-α était présent dans le liquide céphalorachidien à 4 UI/mL tandis que la culture bactériologique du liquide céphalorachidien et la recherche virale par PCR étaient négatives. L'aggravation clinique et biologique dans les 36 premières heures, malgré l'administration d'une triple antibiothérapie parentérale (amoxicilline, ceftriaxone, nétilmicine), puis la réponse clinique et biologique favorable après l'adjonction de la vancomycine orientaient vers une méningite à pneumocoque à sensibilité réduite aux β-lactamines. Une antibiothérapie maternelle par amoxicilline reçue depuis cinq jours et le passage de cet antibiotique dans le lait maternel confortaient l'hypothèse d'une méningite bactérienne décapitée.
Conclusion. - En présence d'une méningite sans germe, la coexistence d'un important syndrome inflammatoire et de la détection de l'IFN-α dans le liquide céphalorachidien doit être considérée comme un phénomène inhabituel et motiver la poursuite de l'antibiothérapie jusqu'à l'identification virale.

Abstract |
Background. - In meningitis without germs, the existence of an inflammatory syndrome leads toward a bacterial etiology while the detection of interferon-alpha (IFN-α ) in the cerebrospinal fluid (CSF) argues for a viral meningitis. The coexistence of the inflammatory syndrome and the presence of IFN-α in the CSF makes this differentiation difficult. The reported case yields the picture and begs the question on the diagnostic approach and the required therapeutic attitude.
Case report. - A six-week-old infant, exclusively breast-fed, was hospitalized for fever. The examination showed an important inflammatory syndrome and meningeal attempt with a cellularity at 94/mm3 with 53% polymorphonuclear neutrophils, contrasting with normal proteinorrhachia and glycorrhachia. The IFN-α in the CSF was present at 4 UI/mL while the bacteriological culture and the viral search by PCR were negative. The clinical and biological worsening within the first 36 hours, in spite of the parenteral dispensation of a triple antibiotic therapy (amoxicillin, ceftriaxone, netilmicin), then a favorable clinical and biological response after adjunction of vancomycin, led toward a pneumococcal meningitis with reduced sensitivity to β-lactams. The maternal antibiotic therapy by amoxicillin and its presence in the maternal milk favored the hypothesis of a decapitated bacterial meningitis.
Conclusion. - In the presence of a meningitis without germs, the coexistence of a sizable inflammatory syndrome and the detection of IFN-α in the CSF must be considered as an unusual phenomenon and motivate the pursuit of antibiotic therapy until viral identification.
