Résumé |
L’année 2012 a été marquée, en cancérologie cutanée, par la consolidation de résultats antérieurs ainsi que l’émergence de nouvelles perspectives thérapeutiques. Les résultats consolidés concernent notamment l’utilisation des inhibiteurs de la voie des MAP-kinases (anti-MEK, anti-BRAF) dans le traitement des patients atteints de mélanome métastatique. Le vismodegib (inhibiteur de la voie SMO) utilisé dans le traitement des carcinomes basocellulaires multiples, inopérables ou développés chez des patients atteints d’un syndrome de Gorlin, a fait l’objet de publication d’études de grande ampleur. Parmi les nouvelles pistes thérapeutiques mentionnons l’utilisation des inhibiteurs ou anticorps monoclonaux dirigés contre le récepteur PD1 ou son ligand PD-L1, la combinaison d’inhibiteurs de MEK et BRAF dans le traitement des mélanomes métastatiques, le traitement en situation adjuvante par ces mêmes inhibiteurs. Au delà de ces avancées significatives à mettre au crédit de la cancérologie cutanée émergent de nouvelles questions relatives aux effets secondaires observés sous ces traitements ciblés. La survenue de carcinomes épidermoïdes multiples ou mélanomes primitifs sous inhibiteurs de BRAF est une illustration frappante des paradoxes consécutifs au transfert clinique des progrès scientifiques.
Les perspectives intellectuelles sont donc nombreuses. Souhaitons qu’elles aboutissent in fine à la mise à disposition des patients de molécules innovantes qu’ils auront contribué à développer grâce à leur participation aux essais cliniques.

In cutaneous oncology, the year 2012 was marked by the consolidation of earlier results as well as the emergence of new therapeutic perspectives. The consolidated results most notably concern the use of MAP-kinase inhibitors (anti-MEK, anti-BRAF) in the treatment of patients with metastatic melanoma. Vismodegib (an SMO inhibitor), used in the treatment of multiple basocellular carcinomas that are inoperable or that have developed in patients suffering from Gorlin syndrome, has been the subject of publication of large-scale studies. Of the new therapeutic advances, let us mention the use of inhibitors or monoclonal antibodies directed against the PD1 receptor and its ligand PD-L1, the combination of MEK and BRAF inhibitors in the treatment of metastatic melanoma, and treatment in adjuvant situations with these same inhibitors. Beyond these significant advances that must be credited to cutaneous oncology, new questions are emerging relative to the side effects observed with these targeted therapies. The onset of multiple epidermoid carcinomas or primary melanomas treated with BRAF inhibitors is a striking illustration of the paradoxes arising from the clinical transfer of scientific progress.
The intellectual perspectives are therefore numerous. It is hoped that this work will succeed in making these innovative drugs available to the patients who will have contributed to their development through their participation in clinical trials.
