Introduction |
Bien que connues depuis longtemps, les infections de la muqueuse buccale par des nématodes du genre Gongylonema sont exceptionnelles, particulièrement en France. Nous en rapportons un nouveau cas.
Observation |
Un homme de 48ans consultait pour des démangeaisons se déplaçant dans la muqueuse buccale depuis quelques semaines. Ce patient vivait en zone rurale dans le sud de l’Alsace et n’avait jamais voyagé en Afrique. Son travail consistait à surveiller le déchargement de farines du port de Bâle. L’examen clinique initial était négatif. Le patient avait pu en revanche photographier un filament présent dans sa muqueuse auparavant, et finalement en extraire un ver. Son identification, réalisée à l’institut de parasitologie de Strasbourg par le Pr Pesson, concluait à un nématode du genre Gongylonema, espèce pulchrum. Aucun autre traitement n’a été jugé nécessaire.
Discussion |
Il est important que les dermatologues connaissent cette parasitose. En effet, sa présentation clinique stéréotypée en permet un diagnostic facile, mais l’absence possible du parasite au moment de la consultation risque de faire errer le diagnostic vers un syndrome d’Ekbom (délire d’infestation parasitaire).

Background |
Although long known, infestations of the buccal mucosa by nematodes of the Gongylonema genus are rare, particularly in France. Herein we report a new case.
Patients and methods |
A 48-year old man consulted for itching in various parts of the buccal mucosa over the preceding weeks. The patient lived in a rural region in southern Alsace and had never visited Africa. His work comprised overseeing the unloading of flour at the port of Basel. The initial clinical examination was negative. However, the patient managed to photograph a filament previously present in his buccal mucosa, and to ultimately extract a worm. The latter was identified at the Strasbourg Institute of Parasitology by Pr Pesson as a nematode of the genus Gongylonema sp. pulchrum. No other treatment was considered necessary.
Discussion |
It is important for dermatologists to be aware of this type of parasitosis. Its stereotyped clinical presentation allows ready diagnosis, but the possible absence of the parasite at the time of the visit may lead to a misdiagnosis of Ekbom syndrome (delusional parasitic infestation).
