Traitement d'une pneumopathie à cytomégalovirus par ganciclovir chez un nourrisson immunocompétent - 01/01/02

Résumé |
L'infection à cytomégalovirus est fréquente chez l'enfant immunodéprimé, où elle est habituellement traitée par ganciclovir, en raison de sa gravité. Elle est également fréquente chez l'enfant immunocompétent, mais habituellement asymptomatique, d'où l'absence d'indication reconnue de ce traitement, qui a par ailleurs une toxicité potentielle.
Observation - Nous rapportons un cas d'infection à cytomégalovirus chez un nourrisson âgé de six semaines, associant une pneumopathie interstitielle et une cassure staturopondérale. Le cytomégalovirus a été mis en évidence dans le lavage bronchoalvéolaire et dans le sang par polymerase chain reaction. L'enfant a été traité par perfusion intraveineuse de ganciclovir pendant 15 jours, avec une bonne efficacité, et n'a pas présenté d'effet indésirable de ce traitement. Il était guéri sans séquelle avec un recul de sept mois et avait un bilan immunitaire normal.
Commentaires - Un traitement intraveineux par ganciclovir peut être proposé, en cas d'infection respiratoire à cytomégalovirus retentissant sur la croissance staturopondérale, avec une bonne efficacité et une bonne tolérance, chez le nourrisson immunocompétent. Cependant d'autres études incluant un plus grand nombre de malades sont souhaitables afin de définir précisément les indications de ce traitement chez l'enfant immunocompétent.

Abstract |
Cytomegalovirus infection is common in the immunodepressed child, and because of its severity, is usually treated with intravenous ganciclovir. It is also common in the immunocompetent child, but is usually asymptomatic, so that there is no indication for ganciclovir treatment, because of the potential toxicity of the drug.
Case report - We report a case of symptomatic cytomegalovirus infection in a six-week-old infant, presenting with interstitial pneumonia associated with weight and height growth failure. Cytomegalovirus was found by polymerase chain reaction in the bronchoalveolar lavage fluid and in the blood. This child was successfully treated with 15 days of intravenous ganciclovir, and experienced no adverse effect of this treatment. The child was free of symptoms and free of immunodeficiency after seven months of follow-up.
Conclusion. - Immunocompetent patients with cytomegalovirus infection, pulmonary involvement and weight and height growth failure, may benefit from intravenous ganciclovir treatment. Further studies, with larger number of cases, are yet needed to define more precisely the indications of such a treatment in the immunocompetent child.
