Le principal facteur de risque de torsion annexielle est un antécédent de torsion (NP3). Il n’existe pas de signe clinique, biologique ou radiologique permettant d’éliminer formellement une torsion (NP3). Une vascularisation au doppler n’élimine pas le diagnostic (NP2). Une cœlioscopie est recommandée en urgence pour détorsion (grade B), sauf chez les femmes ménopausées où une annexectomie est recommandée (grade C). L’absence de recoloration après détorsion n’est pas une indication d’annexectomie systématique (grade C) car une récupération fonctionnelle est possible (NP3). L’ovariopexie systématique n’est pas recommandée (grade C). Les signes cliniques d’hémorragie intrakystique et de rupture du corps jaune ne sont pas spécifiques (NP4). Il n’y a pas d’indication à demander systématiquement une IRM pour confirmer le diagnostic d’hémorragie intrakystique (grade C). La transformation maligne d’un kyste de l’ovaire est très rare. Les femmes présentant un kyste de l’ovaire d’aspect bénin n’ont pas de risque accru de cancer (NP2). Il n’est pas justifié de leur proposer une surveillance échographique systématique (grade C). Les kystes dermoïdes comportant du tissu nerveux peuvent entraîner la synthèse d’auto-anticorps-anti-NMDA aboutissant à une encéphalite. Une forte proportion de tissu thyroïdien dans un tératome mature (struma ovarii) peut s’accompagner d’une hyperthyroïdie.

The main risk factor of adnexal torsion is a previous adnexal torsion (LE3). There is no clinical, biological or radiological sign that may exclude the diagnosis of adnexal torsion (LE3). The presence of flow at color Doppler imaging does not allow exclusion of the diagnosis (LE2). An emergent laparoscopy is recommended for adnexal untwisting (Grade B), except in postmenopausal women where oophorectomy is recommended (grade C). A persistent black color of the adnexa after untwisting is not an indication for systematic oophorectomy (grade C), since a functional recovery is possible (LE3). Ovariopexy is not routinely recommended following adnexal untwisting (grade C). The clinical signs of intra-cystic hemorrhage and those of rupture of the corpus luteum are not specific (LE4). MRI is not recommended to confirm the diagnosis of intra-cystic hemorrhage (grade C). Malignant transformation of an ovarian cyst is very rare. The presence of a benign ovarian cyst is not associated with an increased risk of ovarian cancer at long-term follow-up (LE2). For these women, an ultrasound follow-up is not recommended (grade C). Dermoid ovarian cyst containing nerve tissue can trigger the production of pathogenic auto-antibody-anti-NMDA, leading to encephalitis. A high proportion of thyroid tissue in a mature teratoma (struma ovarii) may cause hyperthyroidism.
