Rétinoblastome : résultats préliminaires du protocole national de prise en charge au CHU de Casablanca - 24/02/14

Le rétinoblastome est le cancer intraoculaire le plus fréquent, survenant quasi-exclusivement chez l’enfant. Nous rapportons les résultats d’une étude prospective évaluant le protocole national de prise en charge du rétinoblastome au Maroc. Le traitement comprenait, selon le stade et la latéralité, la chimiothérapie première soit pour faciliter l’énucléation soit pour rendre un traitement conservateur possible, la chimiothérapie postopératoire, l’énucléation, ainsi que les traitements conservateurs à savoir la thermothérapie transpupillaire, la thermochimiothérapie et la cryothérapie. La radiothérapie a été utilisée dans quelques cas. La surveillance était rapprochée jusqu’à l’âge de 5ans. L’incidence du rétinoblastome pendant la période de l’étude était de 18 nouveaux cas par an dans notre service. Les observations de 32 enfants ont été incluses dans l’étude : 18 rétinoblastomes unilatéraux (56 %) et 14 rétinoblastomes bilatéraux (44 %) soit 46 yeux. La leucocorie était le signe révélateur le plus fréquent (69 %). Une buphtamie ou une exophtalmie étaient présentes dans 47 % des cas. Le rétinoblastome était classé stade V/D ou E (Reese-Elsworth/ABC) dans 69,5 % des cas. Une énucléation a été nécessaire pour 28 yeux. La thermothérapie transpupillaire ou la thermochimiothérapie ont été utilisées pour 13 yeux (11enfants) et une cryothérapie pour 13 yeux (10 enfants). Après un recul moyen de 52 mois, sur 32 enfants, 4 sont décédés et 2 ont abandonné le traitement. Le taux de sauvetage oculaire était de 85,7 %, soit 12 yeux sur 14, obtenu sans radiothérapie dans 11 yeux. Le rétinoblastome est une tumeur d’origine génétique qui se présente sous 2 formes : la forme sporadique toujours unilatérale et la forme héréditaire souvent bilatérale. C’est cette dernière entité qui pose le plus de défi thérapeutique. Les traitements actuels utilisent le plus souvent la chimiothérapie et les traitements locaux. L’avenir est orienté vers les traitements purement locaux notamment par chimiothérapie intra-artérielle ou intraoculaire.

Retinoblastoma is the most frequent intraocular cancer, affecting almost exclusively children. We report prospective study results assessing the national protocol for retinoblastoma treatment in Morocco. Treatment included, depending on stage and laterality, primary chemotherapy either to facilitate enucleation or to make conservative treatment possible, postoperative chemotherapy, enucleation and conservative treatments such as transpupillary thermotherapy, thermochemotherapy and cryotherapy. Radiation was used in a few cases. Close supervision was performed until the age of 5. The incidence of retinoblastoma within the study period was 18 new cases per year in our department. Observations of 32 children were included in the study: 18 unilateral retinoblastomas (56%) and 14 bilateral retinoblastomas (44%), for a total of 46 eyes. Leucocoria was the most frequent presenting symptom (69%). Buphthalmia or proptosis were present in 47% of cases. The stage of retinoblastoma was V/D or E (Reese-Elsworth/ABC) in 69.5% of cases. Enucleation was necessary for 28 eyes. Transpupillary thermotherapy or thermochemotherapy were used for 13 eyes (11 children) and cryotherapy for 13 eyes (10 children). After an average follow-up period of 52 months, among 32 children, 4 died and 2 abandoned treatment. Ocular salvage rate was 85.7% (12 eyes out of 14, among which 11 without radiation). Retinoblastoma is a genetic tumor, which occurs in two forms: sporadic, always unilateral, and hereditary, often bilateral. The latter is the most challenging case. Current treatment protocols rely primarily on chemotherapy and local treatments. The future is oriented toward purely local treatments such as intra-arterial chemotherapy and intraocular chemotherapy.
