Contraception des femmes allaitantes : place des spermicides - 19/12/14

Toute méthode contraceptive prescrite à une femme allaitante doit non seulement être adaptée à son état physiologique après l’accouchement et à la santé de son nouveau-né mais également correspondre à ses attentes. Selon les recommandations françaises, la contraception hormonale combinée est proscrite jusqu’à 6 semaines après l’accouchement chez la femme allaitante et elle n’est pas recommandée dans les 6 mois. Les dispositifs intra-utérins et les progestatifs ne peuvent être utilisés qu’à partir de la quatrième semaine après l’accouchement. Les spermicides bénéficient d’une indication privilégiée chez les femmes allaitantes. Ils ne rencontrent en effet aucune restriction dans les recommandations nationales et internationales. En France, seuls les spermicides à base de chlorure de myristalkonium ou de chlorure de benzalkonium qui ne passent pas dans le lait maternel selon Liebert [31 ] sont utilisés. En plus de leur sécurité d’utilisation, l’effet lubrifiant de certaines formes galéniques des spermicides peut être utile. Les spermicides peuvent être utilisés conjointement avec un préservatif ainsi qu’avec la méthode de l’allaitement maternel et de l’aménorrhée (MAMA). L’acceptabilité et le succès des spermicides chez la femme allaitante dépendent très largement de l’information reçue par l’utilisatrice, de sa motivation et de sa bonne compréhension des conditions d’utilisation.

Any contraceptive method prescribed to a breastfeeding woman should be not only adapted to her physiological condition after delivery and to the health of her newborn, but should also meet her expectations. According to the French guidelines, combined hormonal contraception is prohibited up to 6 weeks after delivery in breastfeeding women and is not recommended within 6 months. Intrauterine devices and oral progestin alone may be utilized only from the fourth week after delivery. Spermicides have a privileged indication in breastfeeding women. Indeed, they have no restriction in national and international guidelines. In France, spermicides primarily include myristalkonium chloride and benzalkonium chloride that does not pass into maternal milk according Liebert [31 ]. In addition to their safe use, the lubricant effect of some specialties can be useful. Spermicides can be used in combination with a condom as well as with the lactational amenorrhea method. The acceptability and success of spermicides in breastfeeding woman depends largely on the information of users, their motivation and understanding of the conditions of use.
