Analyse de la macula par OCT spectral domain dans la chirurgie du décollement de rétine rhegmatogène - 19/03/15

Introduction |
Malgré une réapplication intégrale des couches rétiniennes après chirurgie de décollement de rétine rhegmatogène (DRR), les résultats fonctionnels postopératoires peuvent être limités. Ceci s’explique par les modifications maculaires microstructurelles inhérentes à la pathologie elle-même ainsi qu’à la chirurgie.

Étudier les différentes modifications de la macula par tomographie en cohérence optique en mode spectral domain (OCT-SD) en pré- et postopératoire et les corréler aux résultats fonctionnels et cliniques chez les patients présentant un DRR. Établir les facteurs pronostiques pré- et postopératoires.
Patients |
Il s’agissait d’une étude prospective portant sur 50 yeux de 50 malades opérés pour DRR.
Méthodes |
Chaque patient avait bénéficié d’un examen clinique complet ainsi qu’une OCT-SD de la région maculaire grâce à l’OCT Spectralis Heidelberg ; en préopératoire puis successivement à j7, 1 mois, 3 mois et 6 mois après chirurgie du DRR.
Résultats |
Les résultats tomographiques préopératoires avaient objectivé une hauteur moyenne de liquide sous rétinien (LSR) de 742±345μm. Quatre-vingt-seize pour cent des patients présentaient un épaississement des segments externes des photorécepteurs (PROS), 62 % une ondulation de la rétine externe, 60 % des cavitations des couches nucléaires externes et internes (CNE, CNI), 36 % une déstructuration de la membrane limitante externe (MLE) et 64 % une déstructuration de la ligne de jonction IS/OS. En postopératoire, 24 % présentaient une persistance de LSR. Les épaisseurs moyennes : fovéale centrale, de la CNE, de la ligne IS/OS et de la PROS étaient respectivement de 172±51,3μm, 88,4±30,9μm, 11,36±5,4μm et 19,54±3,1μm. Des déstructurations postopératoires de la MLE, de la ligne IS/OS et de la membrane de Verhoeff (VM) étaient présentes respectivement chez 24 %, 60 % et 82 % des patients. Les facteurs de risque tomographiques préopératoires de mauvaise récupération visuelle étaient : la hauteur du LSR>800μm (p<0,001), la déstructuration de la MLE et/ou de la ligne IS/OS (p<0,001), ainsi que la présence de cavitations dans les CNE et/ou CNI (p=0,002). Les facteurs de risque postopératoires étaient : la diminution des épaisseurs fovéale moyenne (≤250μm), fovéale centrale (≤160μm), de la CNE (≤90μm), de la PROS (≤18μm) et/ou de la ligne IS/OS (≤10μm) (p<0,001) et enfin, l’aspect discontinu ou absent de la MLE (p<0,001), de la ligne IS/OS (p<0,001) et de la VM (p=0,006).
Conclusion |
L’OCT-SD permet d’objectiver des anomalies rétiniennes maculaires microscopiques quasi histologiques spécifiques du décollement de rétine rhegmatogène. Ces anomalies seraient prédictives de la récupération visuelle postopératoire finale.

Introduction |
Despite surgical reattachment of retinal layers, postoperative functional outcomes after rhegmatogenous retinal detachment (RRD) may be limited. This can be explained by microstructural changes in the macula inherent to the pathology itself as well as the surgery.
Purpose |
To evaluate the various changes in the macula by OCT pre- and postoperatively, and correlate them with functional and clinical outcomes in patients with RRD. To establish pre- and postoperative prognostic factors.
Methods |
This was a prospective study of 50 eyes of 50 patients operated for RDD. Each patient underwent a complete clinical examination and macular OCT using the Heidelberg Spectralis; preoperatively and then successively at 7 days, 1 month, 3 months and 6 months after RRD surgery.
Results |
Preoperative tomographic results revealed an average height of submacular fluid of 742±345μm. Ninety-six percent of patients exhibited thickening of photoreceptor outer segments (PROS), 62% an outer layer undulation, 60% cystic cavities in the outer and/or inner nuclear layers (ONL, INL), 36% disruptions of the external limiting membrane (ELM) and 64% disruption of the IS/OS junction. Postoperatively, 24% of patients had persistent submacular fluid. The average thickness of the central fovea, the ONL, the IS/OS junction and the PROS were 172±51.3μm, 88.4±30.9μm, 11.36±5.4μm and 19.54±13.1μm respectively. Postoperative disruptions of the ELM, the IS/OS junction and Verhoeff's membrane (VM) were present in 24%, 60% and 82% of patients respectively. The preoperative tomographic risk factors for poor visual outcome were: submacular fluid height>800μm (P<0.001), disruptions of the MLE and/or IS/OS junction (P<0.001), as well as cystic cavities in the ENL and/or INL (P=0.002). Postoperative risk factors were: thinning of the fovea (≤250μm), central fovea (≤160μm), ONL (≤90μm), IS/OS junction (≤ 10μm) and PROS (≤18μm) layers (P<0.001), as well as a discontinuous or absent appearance of the ELM (P<0.001), IS/OS junction (P<0.001) and VM (P=0.006).
Conclusion |
Spectral domain OCT allows detection of specific microscopic, quasi-histologic macular changes in rhegmatogenous retinal detachment. These anomalies could be predictive of final postoperative visual outcome.
