Résumé |
Contexte |
La confiance en soi et le sentiment d’auto-efficacité des mères dans leur capacité à allaiter leur nouveau-né sont reconnus comme faisant partie des facteurs positivement associés à l’instauration et à la durée de l’allaitement maternel.
Objectif |
Évaluer l’importance de cette association à l’aide de la Breastfeeding Self-Efficacy Scale (BSES), échelle d’auto-évaluation ponctuelle du sentiment d’auto-efficacité dans l’allaitement maternel.
Méthodes |
Cette étude prospective a été réalisée en 2012 à la maternité Jeanne-de-Flandre du centre hospitalier régional universitaire (CHRU) de Lille (France). Durant leur séjour en maternité, les mères allaitantes incluses ont complété la BSES, et ont fait l’objet d’un suivi téléphonique à un et à trois mois du post-partum.
Résultats |
Cent quarante-neuf mères ont été incluses. Le taux d’allaitement était de 86,5 % à 1 mois et 60 % à 3 mois. Le score BSES des mères poursuivant l’allaitement à 1 et 3 mois était significativement supérieur à celui des mères ayant arrêté l’allaitement, avec une aire sous la courbe ROC (receiver operating characteristics) de 0,72 à 3 mois, confirmant le caractère performant de la BSES pour la prédiction de la poursuite de l’allaitement. Le score BSES des multi-allaitantes était significativement supérieur à celui des primo-allaitantes. Une valeur seuil du score BSES a été déterminée à 116 sur 165.
Conclusion |
Il est nécessaire que les mères peu confiantes en elles quant à leur allaitement soient dépistées dès le séjour en maternité ou même avant. L’intérêt d’interventions de soutien à l’allaitement reposant sur l’utilisation de la BSES nécessite d’être évalué au moyen d’études randomisées.

Context |
Maternal self-confidence and self-efficacy in breastfeeding are recognized as factors positively associated with the initiation and duration of breastfeeding.
Objective |
To evaluate the importance of this association using the Breast Feeding Self-Efficacy Scale (BSES).
Method |
This prospective study was conducted in 2012 in the Jeanne-de-Flandre maternity department in the Lille University Hospital (France). During their time in the maternity department, breastfeeding mothers who participated in the study completed the BSES, a brief self-assessment of their feelings of self-efficacy relating to breastfeeding. They then received follow-up telephone interviews at 1 and 3 months postpartum.
Results |
One hundred and forty-nine mothers were included in the study. Breastfeeding rates were 86.5% at 1 month and 60% at 3 months. The BSES score of mothers who continued to breastfeed at 1 and 3 months was significantly higher than the score of mothers who had already weaned their children, with an AUROC of 0.72 at 3 months. This confirmed the reliability of the BSES for predicting adherence to breastfeeding. The BSES score of mothers who had previously breastfed was significantly higher than for those breastfeeding for the first time. The threshold score for the BSES was determined as 116/165.
Conclusion |
It is important that mothers who lack confidence in their ability to breastfeed be identified early, whether on the maternity ward or even before this point. The value of BSES-based breastfeeding support intervention needs to be evaluated through randomized trials.
