Évolution d’un infundibulum de l’artère communicante postérieure jusqu’à un anévrysme et sa rupture |
L’infundibulum est une dilatation à l’origine d’une artère. Il siège principalement à l’origine de l’artère communicante postérieure. Sa fréquence est estimée de 7 à 25 % selon les auteurs. Peu de travaux décrivent le développement d’un anévrysme et la rupture d’un infundibulum préalablement découvert par angiographie cérébrale. Nous présentons les cas de deux patients ayant eu une hémorragie méningée récidivante.
Dans les deux cas, une angiographie cérébrale des 4 axes, réalisée à la suite de l’hémorragie initiale, avait révélé un infundibulum de l’artère communicante postérieure. Neuf et onze ans plus tard, une seconde hémorragie méningée amenait à la ré-hospitalisation des deux patients : cette fois, l’angiographie cérébrale montrait un véritable anévrysme à l’endroit où se trouvait autrefois l’infundibulum. Les deux patients étaient alors opérés avec un bon résultat post-opératoire.
Ce travail ajoute deux nouveaux cas à la littérature et montre qu’un infundibulum peut évoluer et se transformer en anévrysme. Ceci devrait nous obliger à suivre très régulièrement les patients ayant un infundibulum, surtout ceux qui ont eu une hémorragie méningée, afin que l’anévrysme soit découvert à temps.

A small dilatation known as infundibular widening (IFw) is frequently seen (7%-25%) on the posterior communicating artery (PComA) at its origin from the internal carotid artery. Development and subsequent rupture of an aneurysm on a previously radiographically demonstrated IFw has rarely been documented.
We present two patients who suffered from subarachnoid hemorrhage (SAH). Initial cerebral angiography demonstrated IFw on PComA. They were readmitted to the hospital 9 and 11 years later, after a new SAH. Repeated cerebral angiography revealed an aneurysm arising from the site where the IFw had been seen previously in both cases. The aneurysms were clipped with favorable outcome.
This report adds two new cases documenting enlargement of PComAIFw into an aneurysm. Patients with PComAIFw, especially those who have experienced SAH, should be considered for periodic follow-up to rule out the development of an aneurysm over time.
