Une neuropathie optique bilatérale survenant chez un sujet jeune doit faire suspecter une origine toxique ou héréditaire. Nous rapportons le cas d’un homme, âgé de 38 ans, présentant une neuropathie optique bilatérale. L’acuité visuelle était réduite à la perception lumineuse à l’œil droit et à 2/10e à l’œil gauche. L’anamnèse révéla une dépendance à l’héroïne. Trois molécules furent suspectées chez ce patient : l’héroïne, la quinine et la cocaïne. L’exploration et une revue de la littérature permirent d’évoquer la responsabilité de l’héroïne. Ce cas clinique permet de souligner l’importance d’une anamnèse exhaustive, reprenant notamment une dépendance à l’héroïne ou à la cocaïne, devant toute neuropathie optique bilatérale chez un sujet jeune.

Bilateral optic neuropathy in a young man is suggestive of hereditary or toxic damage. We describe the case of a 38-year-old man with bilateral optic neuropathy. His best corrected visual acuity was light perception OD and 20/125 OS. The patient’s history revealed an addiction to heroin. Three drugs — heroin, quinine, and cocaine — were considered as a possible cause, which is discussed further. We suggest that the condition reported here may be attributed to the use of heroin. This underlines the importance of meticulous history taking in diagnosing a young man with bilateral loss of vision.
