Plasmocytome cutané EBV+ isolé chez une malade transplantée rénale : efficacité d'un traitement par rituximab - 29/04/08

Introduction |
Les syndromes lymphoprolifératifs post-transplantation sont des complications bien connues de la transplantation d'organe habituellement associées au virus Epstein-Barr (EBV).
Observation |
Une femme de 25 ans, transplantée rénale, avait deux nodules sous-cutanés sur le mollet apparus 3 ans après une seconde greffe rénale. La biopsie d'un nodule révélait un plasmocytome associé à l'EBV localisé dans l'hypoderme. Le bilan d'extension ne montrait pas de localisation extra cutanée. Un traitement par rituximab était débuté et permettait une rémission complète.

Les localisations extra-ganglionnaires des syndromes lymphoprolifératifs post-transplantation sont régulièrement rapportées, mais les localisations cutanées isolées restent exceptionnelles. Les présentations histologiques sont très variées, mais la forme à type de plasmocytome est rare. Le traitement initial des syndromes lymphoprolifératifs post-transplantation cutanés, liés à l'EBV, sans atteinte extracutanée associée, consiste le plus souvent en une réduction de l'immunosuppression et/ou un traitement antiviral et d'une surveillance prolongée. L'utilisation d'anticorps monoclonaux anti-CD20 (rituximab) est actuellement proposée.

Introduction |
Postransplant lymphoproliferative disorders are well known complications of solid organ transplant, usually associated with Epstein-Barr virus (EBV).
Observation |
A 25 year old renal transplant patient presented with two subcutaneous nodules on the lower limb that appeared 3 years after a second renal transplantation. Biopsy of one nodule showed an EBV associated plasmocytoma located in the subcutaneous tissue. A complete systemic evaluation showed no evidence of extracutaneous involvement. The patient was treated with anti CD20 therapy (rituximab), and complete remission was achieved.
Discussion |
Extranodular localisations of postransplant lymphoproliferative disorders are usually reported, but cutaneous localizations are rarely described. Histological presentation are various, but plasmocytoma-type is infrequent. Initial therapy of cutaneous EBV-associated postransplant lymphoproliferative disorders without extracutaneous involvement consists in reduction of the immunosuppression therapy and/or an antiviral treatment and prolonged surveillance. Treatment with monoclonal anti-CD20 antibodies (rituximab) is proposed.
