Tumeurs primitives des voies lacrymales : pièges diagnostiques et thérapeutiques - 12/04/13

Introduction |
Moins de 250 tumeurs primitives des voies lacrymales sont rapportées dans la littérature internationale. Leurs signes cliniques, pauvres et peu spécifiques, sont souvent responsables d’un retard au diagnostic. Le traitement des tumeurs malignes épithéliales est chirurgical éventuellement suivi de radiothérapie.
Patients et méthodes |
Il s’agit d’une étude rétrospective portant sur cinq patients porteurs d’une tumeur maligne des voies lacrymales.
Résultats |
Trois patients présentaient un carcinome relevant d’un traitement chirurgical en monobloc avec reconstruction. Une radiothérapie postopératoire a été réalisée dans un cas. Une patiente a présenté une évolution locale et ganglionnaire concomitante à trois ans pour laquelle elle a bénéficié d’un traitement curatif. Deux autres patients présentaient un lymphome malin non hodgkinien B diffus à grandes cellules.
Discussion |
Les symptômes précoces des tumeurs des voies lacrymales sont peu spécifiques et évoquent une dacryocystite. La réalisation d’un dacryoscanner, d’une IRM et d’une rhinoscopie peut permettre un diagnostic précoce. Le traitement chirurgical requiert de larges marges d’exérèse et une reconstruction de la région canthale interne.
Conclusion |
La précocité du diagnostic limite les séquelles fonctionnelles secondaires au traitement chirurgical, l’importance de la reconstruction et améliore la survie.

Introduction |
Fewer than 250 primary tumors of the lacrimal duct have been reported in the international literature. Because their signs and symptoms are nonspecific and usually subclinical, delayed diagnosis is common. Treatment for malignant epithelial tumors is surgical, with or without radiation.
Patients and methods |
The present study is a retrospective analysis of five patients with malignant lacrimal duct tumors.
Results |
Three patients were diagnosed with carcinoma requiring wide en bloc surgical resection with surgical reconstruction. One underwent adjuvant radiation therapy. Another experienced simultaneous local and lymph node recurrence after 3years and underwent curative surgery. Two additional patients were diagnosed with diffuse malignant large B-cell non-Hodgkins lymphoma.
Discussion |
Early symptoms of lacrimal duct tumors are nonspecific and mimic acute dacryocystitis. Lacrimal system CT, MRI and rhinoscopy may allow for earlier diagnosis. Surgical treatment requires resection with wide margins and reconstruction of the medial canthal region.
Conclusion |
Only an early diagnosis can reduce the impairment related to surgery and the extent of the required reconstruction, as well as improve survival.
