Arterial involvement is rare in Behçet's disease but can be at the forefront of the clinical picture and cause life-threatening complications. Case reports. A 36-year-old man had Behçet's disease with an aortographically documented aneurysm of the abdominal aorta as the inaugural manifestation. He had oral and genital ulcers. Funduscopy showed periphlebitis. In a 38-year-old man with an 8-year history of Behçet's disease, pulmonary and coronary artery aneurysms developed, as well as intracardiac and venous thromboses. Discussion. Arterial involvement occurs in 3-5% of patients with Behçet's disease and usually manifests as multiple spindle-shaped aneurysms. Intracardiac thrombosis and cardiac aneurysm are exceedingly rare. Our patient had an extremely unusual presentation given the low rate of occurrence of arterial lesions in Behçet's disease. Conclusion. Arterial involvement in Behçet's disease raises treatment challenges because the lesions tend to recur and can cause life-threatening complications.

Arterial involvement is rare in Behçet's disease, it may occasionally dominate the clinical presentation and in some cases quickly involve the vital prognosis. Patients. Case 1. B.A., 36 years old developed Behçet disease revealed by aneurysm aorta abdominal which had been confirmed by the aortography. The diagnosis has been settled by the notion of past bipolar aphtous ulceration and the ocular involvement which included periphlebitis. Case 2. A case of pulmonary arterial aneurysm with cardiac and venous thrombosis in a 38 year old patient with 8 years history of Behçet disease is reported. Discussion. Arterial involvement in Behçet disease is observed in 3 to 5% of cases. It is frequently consisting in multiple aneurysm. Cardiac involvement including thrombosis and aneurysm is rare. The unusual onset of a disease in one case by an arterial involvement and the rarity of arterial manifestations in the second constitute the originality of our work. Conclusion. Cardiovascular manifestations of Behçet's disease pose therapeutic problems because of their recurrent character which may involve vital prognosis.
