Le syndrome canalaire du genou peut concerner 3 nerfs : le nerf tibial, le nerf saphène ou le nerf fibulaire. Il doit être recherché de façon systématique. Le diagnostic est clinique, basé sur l’interrogatoire (douleur neuropathique) et le trajet anatomique. L’atteinte du nerf fibulaire peut entraîner une paralysie des extenseurs des orteils et de la cheville avec une hypoesthésie de la face dorsale du pied. Celle du nerf tibial se manifeste par une douleur du creux poplité qui augmente lors de la dorsiflexion du pied. Une douleur de la partie interne de la cuisse évoque plutôt une atteinte du nerf saphène, notamment lorsqu’elle augmente à l’extension contrariée du genou. Une electroneuromyographie recherche le site de compression et évalue la sévérité mais elle est souvent normale. Une échographie (voire une IRM) avec un opérateur expérimenté pourra rechercher une lésion nerveuse ou compressive. Le traitement repose sur des antalgiques, comprenant les traitements de la douleur neuropatique (gabapentine), l’éviction des facteurs favorisants. En cas d’échec, une infiltration échoguidée pourra être proposée. La chirurgie reste une indication rare.

The entrapment neuropathy of the knee can affect three nerves: the tibial nerve, the saphenous nerve or the peroneal nerve. It must be systematically searched. The diagnosis is clinical, based on the examination (neuropathic pain). The peroneal nerve damage may cause paralysis of the extensors of the toes and ankle with hypoesthesia of the dorsal foot. The entrapment of the tibial nerve is manifested by popliteal pain which increases with dorsiflexion of the foot. The inner pain of the thigh can be caused by entrapment of the saphenous nerve, particularly when it increases to upset knee extension. The electroneuromyography searches the compression site and assesses the severity but is often normal. An ultrasound (sometimes an MRI) with an experienced operator can seek a nervous or compressive lesion. Treatment consists of analgesics, including treatments for neuropathic pain (gabapentin), the eviction of the contributing factors. If unsuccessful, an ultrasound-guided infiltration could be proposed. The surgery is uncommon.
