Introduction |
La colonisation des cathéters vasculaires centraux par Malassezia sp. est à l'origine de certaines fongémies sur cathéters chez des adultes recevant des intralipides. Les patients d'oncohématologie semblent à risque de cette complication car ils bénéficient de cathéters pour une longue durée et reçoivent souvent des intralipides. Toutefois, la colonisation des cathéters est probablement sous-estimée dans la mesure où l'emploi de milieux spécifiques est rarement réalisé.

Rechercher la fréquence de colonisation des cathéters vasculaires par Malassezia sp. dans une population de patients adultes hospitalisés dans un service d'onco-hématologie et la comparer à celle d'une population de patients adultes hospitalisés en réanimations médicale et chirurgicale.
Matériels et méthodes |
Etude prospective de 100 cathéters vasculaires adressés au laboratoire de microbiologie. Les cathéters étaient rincés dans 1 ml de sérum physiologique et ensemencés sur des milieux de bactériologie classique et des milieux spécifiques pour Malassezia (Leeming et Dixon).
Résultats |
Sur 100 cathéters étudiés, 11 étaient des cathéters périphériques sans aucune colonisation par Malassezia qui ont été exclus de l'étude. Parmi les 89 cathéters vasculaires centraux, cinq cathéters (5,6 %) étaient colonisés par Malassezia sp. Un cathéter sur 48 (2 %) provenait du service d'hématologie, et 4 sur 52 (8 %) provenaient du service de réanimation réalisé. Sur les 4 cathéters de réanimation, 3 d'entre eux étaient aussi colonisés par des bactéries. Aucun des 5 patients porteurs d'un cathéter colonisé par Malassezia n'avait reçu d'intralipides.
Discussion - Conclusion |
La colonisation des cathéters vasculaires par Malassezia sp. chez l'adulte existe dans une proportion non négligeable (5,6 %). Cette colonisation n'est pas liée à la prescription d'intralipides. Des études supplémentaires seraient nécessaires pour évaluer la signification clinique de ces données et étudier la participation de Malassezia sp. à la constitution du biofilm sur les cathéters.

Background |
The colonization of intravascular catheters with the lipo-dependant yeast Malassezia is the cause of vascular catheter-associated fungemia in adults receiving intralipids. Patients with hematological malignancies seem to be at risk for this complication as they beneficiate from long- term catheters and are often given intralipids. However, the colonization of the catheters was probably underestimated as the culture media used in routine are not favorable for the growth of Malassezia .
Aim of the study |
To evaluate the frequency of intravascular catheters colonization with Malassezia in adult patients from a hematology ward and to compare with the frequency observed in adults patients hospitalized in intensive care units.
Materials and methods |
Prospective study of 100 catheters sent to the microbiology laboratory. The catheters were washed in 1 ml saline. Routine bacteriological media and Malassezia specific media (Dixon and Leeming) were seeded with 200 μl cach.
Results |
Among the 100 catheters studied, 11 were peripheral, did not grow Malassezia , and were excluded from the analysis. On the 89 central lines, 5 (5.6 %) grew Malassezia : 1 out of 48 (2 %) in the hematology ward, and 4 oust of 52 (8 %) in the intensive care units. Of the four colonized catheters in ICU, three of them were also colonized with bacteria. None of the five patients with Malassezia positive catheters were given intralipids.
Discussion - Conclusion |
Colonization of central vascular catheters with Malassezia is not negligable and don't bind to intra-lipids prescription. Others studies are warranted to evaluate the clinical significance of Malassezia colonization and to know the role of this yeast in the biofilm built-up on the catheter tips.
