Mucormycose rhinocérébrale chez un diabétique révélée par une extraction dentaire - 04/03/08

Il s'agit d'un cas de mucormycose rhinocérébrale d'évolution subaiguë révélatrice d'un diabète. Le patient a été hospitalisé au décours d'une extraction dentaire, pour une infection sévère de la face, compliquée d'une thrombose du sinus caverneux, puis de la carotide interne, malgré un traitement anticoagulant efficace. Après 7 semaines d'antibiothérapie, il persistait toujours un abcès orbitaire. Les prélèvements bactériologiques et mycologiques de l'abcès étaient négatifs, mais l'examen histologique a permis de porter le diagnostic de mucormycose en montrant la présence de filaments mycéliens. L'évolution a été progressivement favorable sous un traitement prolongé par amphotéricine B, 2 interventions chirurgicales et antivitamine K.

We report a case of subacute rhinocerebral mucormycosis revealing diabetes in a 51-year-old man. He was admitted after a dental extraction for a deep fascial space infection, complicated with cavernous sinus thrombosis. Despite treatment with antibiotics, insulin and heparin, he developed thrombosis of the left internal carotid artery. After 7 weeks of treatment with antibiotics, a computed tomography of the face still showed an abscess of the left orbit. Cultures for bacteria and fungi remained sterile, but fungal hyphae were found on a biopsy specimen that confirmed the diagnosis of mucormycosis. The patient's state improved progressively with prolonged intravenous amphotericin B, 2 operations and anticoagulant.
