Objectif |
Evaluer l'utilisation des antifongiques systémiques ; comparer les indications et les molécules utilisées à celles préconisées dans les recommandations élaborées par le groupe de travail.
Méthodologie |
1) Rédaction de recommandations à partir des données de la littérature et de l'expérience des spécialistes. Proposition d'un arbre décisionnel avec une classification simplifiée.
2) Etude : Cohorte prospective exhaustive de janvier à juin 2001 (adultes et enfants traités par antifongiques systémiques). Recueil de la thérapeutique antifongique prescrite et des éléments de classement et de jugement de l'indication.
Résultats |
Cent vingt et un patients ont été traités pendant cette période. Il s'agissait dans tous les cas de patients immunodéprimés ou à haut risque d'infection fongique invasive dans le cadre de leur pathologie sous-jacente. Les antifongiques systémiques prescrits étaient l'amphotéricine B (AMB) dans 26 % des cas, l'amphotéricine B liposomale dans 14 %, le fluconazole (FCZ) dans 54 %. Une adaptation était effectuée dans 29 % des cas, en général en raison d'une néphrotoxicité ou d'une évolution défavorable. Suivant les indications, les traitements prescrits étaient : 21 % en chimioprophylaxie primaire dont 88 % de FCZ ; 36 % en traitement empirique avec utilisation en proportions équivalentes de l'AMB et du FCZ ; 42 % en traitement curatif avec 65 % de FCZ dans les infections à levures et 100 % d'AMB dans les infections à champignons filamenteux.
Conclusion |
Cette étude montre une bonne conformité des prescriptions aux recommandations. L'antifongique le plus souvent prescrit en hématologie qu'il s'agisse de prophylaxie, de traitement empirique ou curatif est l'AMB. En l'absence de neutropénie prolongée ou de critères de gravité dans le cadre des candidoses, l'antifongique de choix reste le FCZ.

Objective |
The aims of this study were to evaluate the use of intravenous antifungal agents and to compare the strategies and the drugs used with those recommended in the guidelines of our hospital.
Method |
1) We established recommendations issued from the data of the published guidelines and the experience of the local specialists. We proposed a decisional guide with a simplified classification.
2) A prospective study was conducted from January to June 2001 including all adults and children treated with intravenous antifungal agents. Collection of prescribed antifungal therapy and clinical data allowed a classification and judgement of the strategy (i.e. disease prevention, empirical and curative therapy).
Results |
One hundred twenty-one patients were treated during this period. All the patients were at high risk of developing invasive fungal infections because of their underlying disease.
The antifungal drugs prescribed were amphotericin B (AMB) in 26% of the cases, liposomal amphotericin B in 14% and fluconazole (FCZ) in 54%. Therapy was modified in 29%, in general because of nephrotoxicity or unfavorable course. The treatments were prescribed in 21% of cases for disease prevention (including 88% of FCZ), in 36% of cases as empirical treatment with similar use of AMB and FCZ and finally in 42% the antifungal drugs were prescribed as curative treatment using FCZ in 65% of candidosis and AMB in all filamentous fungal infections.
Conclusion |
This study shows that the choice of the drugs was well adapted to the recommendations. The drug generally prescribed in hematology for disease prevention, empirical or curative treatment was AMB. In the absence of prolonged neutropenia and in the absence of criteria of candidosis severity, FCZ remained the drug of choice.
