Invasive pulmonary aspergillosis in a neonate treated successfully with amphotericin B and itraconazole. A case report - 04/03/08

Un cas d'aspergillose pulmonaire invasive chez un nouveau-né traité avec succès par amphotéricine B et itraconazole |
Un cas rare d'aspergillose pulmonaire invasive dû à Aspergillus flavus chez un nouveau-né d'un mois est rapporté. Le diagnostic a été effectué par la mise en évidence de filaments hyalins, septés et ramifiés sous KOH dans le liquide de lavage bronchiolo-alvéolaire, par examen histopathologique d'une masse protubérante dans la bronche droite, et par isolement répété d'Aspergillus flavus en culture. Les principaux facteurs favorisants étaient représentés à la fois par une combinaison variable d'immunodépression et d'hypoglobulinémie infantile transitoire, et l'administration d'antibiotiques à large spectre pour traiter des épisodes récurrents d'infections bactériennes. L'enfant a été traité avec succès par une association d'amphotéricine B par voie intra-veineuse et d'itraconazole per os, ainsi que par la résection partielle de la masse protubérante dans la bronche droite par [wire basket] et aspiration bronchoscopique. Il n'y a pas eu de récidive après un an de recul.
À notre connaissance, il s'agit du premier cas d'aspergillose pulmonaire invasive chez un nouveau-né sur terrain d'immunodépression et d'hypoglobulinémie infantile transitoire, dont la survie peut être rapportée au diagnostic précoce et au traitement par amphotéricine B et itraconazole.
A rare case of invasive pulmonary asp ergillosis in a one- month-old neonate caused by Aspergillus flavus is reported. The diagnosis was based upon repeated demonstration of hyaline, septate branched hyphae in KOH wet mounts of bronchoalveolar lavage, histopathologic examination of a tissue mass protruding into the right main bronchus and repeated isolations of Aspergillus flavus in culture. The main predisposing factors were combined variable immunodeficiency/transient hypoglobulinemia of infancy and repeated administration of broad spectrum antibiotics used to treat recurrent episodes of bacterial infections. The neonate was treated successfully with combined therapy of intravenous amphotericin B deoxycholate and oral itraconazole aided by partial removal of the infected tissue mass protruding in the right main bronchus by wire basket and by bronchoscopic suction. He had no relapse during a follow up of about 1 year. As far as we are aware, this is the first case of invasive pulmonary aspergillosis in a neonate with combined variable immunodeficiency/transient hypoglobulinemia of infancy who also survived the infection due to timely diagnosis and therapy with amphotericin B and itraconazole.
