LA CASPOFUNGINE DANS LE TRAITEMENT EMPIRIQUE DES INFECTIONS FONGIQUES - 04/03/08

Un essai randomisé, multicentrique, en double aveugle, portant sur 1 123 patients, a été réalisé pendant une période de 32 mois par 116 centres investigateurs appartenant à 26 pays. Il avait pour but d’évaluer la caspofungine versus l’amphotéricine B liposomale dans le traitement empirique des infections fongiques présumées chez les patients adultes neutropéniques fébriles.
Le succès thérapeutique a été évalué à partir d’un critère composite. Une réponse favorable devait satisfaire chacun des cinq critères le composant, à savoir : succès thérapeutique de l’infection fongique initiale, absence de survenue d’infection fongique au cours du traitement ou dans les 7 jours après la fin du traitement, survie à 7 jours après la fin du traitement de l’étude, absence de sortie d’étude due à une toxicité liée à ce médicament ou à un manque d’efficacité, disparition de la fièvre au cours de la période de neutropénie.
Les résultats montrent que, dans cette étude, la caspofungine a été aussi efficace que l’amphotéricine B liposomale. Les résultats obtenus par la caspofungine sur les infections initiales et sur l’absence d’arrêt prématuré du traitement pour effets indésirables ou manque d’efficacité sont significativement meilleurs.
Enfin, la caspofungine est nettement mieux tolérée que l’amphotéricine B liposomale, quels que soient les critères analysés.
Caspofungin in the empirical treatment of fungal infections |
A randomized, double blind, multicenter study of 1,123 patients, was conducted over a period of 32 months in 116 research centers in 26 different countries. The study was aimed at comparing caspofungin versus liposomal amphotericin B in the empirical treatment of suspected fungal infections in neutropenic patients with fever.
Therapeutic efficacy was assessed on a composite criterion. A favorable response had to fulfill each of the five criteria: therapeutic efficacy on the initial fungal infection, absence of the occurrence of fungal infection during treatment or in the 7 days following the end of treatment, survival for 7 days after the end of treatment, absence of drop-out from the study due to toxicity related to the drug or because of inefficacy and the disappearance of fever during the neutropenic period.
The results show that in this study caspofungin was as efficient as liposomal amphotericin B. The results obtained with caspofungin on the initial infections and the absence of early withdrawal from treatment due to side effects or lack of efficacy were significantly better.
Furthermore, tolerance to caspofungin is far better than to liposomal amphotericin B, whatever the criteria analyzed.
