How to improve the management of mentally disabled patient before anaesthesia

Résumé |
La prise en charge anesthésique d’un patient handicapé mental est difficile et complexe au bloc opératoire. C’est la phase précédant l’endormissement qui est la plus délicate à gérer du fait des difficultés de communication, de compréhension et de contrôle émotionnel du patient. Afin d’optimiser la prise en charge, des acteurs du milieu médico-éducatif et du domaine anesthésique, ont été audités afin de connaître leur perception de cette relation soignante particulière et les propositions qui seraient susceptibles de l’améliorer. L’accent a été mis sur la formation initiale et complémentaire, la prise en compte des proches, une connaissance de l’individu avant celle de sa pathologie, la transmission de ces éléments entre professionnels concernés, l’accueil éventuel d’un proche en salle d’intervention jusqu’à la perte de connaissance et plus tard en salle de surveillance post-interventionnelle.

The anaesthetic management of mentally disabled patient is difficult. The period of time before the induction of anaesthesia, is especially complex because of low communication, understanding and emotional control with possible violent reactions. The authors have interviewed healthcare professionals and anaesthetists who take care of these patients, about their feelings, difficulties and suggestions for a better management. They have highlighted the value of a good specific knowledge about the mental disability, of the collaboration between health care providers and others, and the need to evaluate the personality, capacities and behaviour of the patients. To let the relatives close to the patient in the operating room could be helpful to relieve his anxiety and let him feeling secure and more cooperative.
