Le CANVAS (Cerebellar Ataxia, Neuropathy, Vestibular Areflexia Syndrome) est un syndrome neurologique rare qui comporte d’une neuropathie sensitive type neuronopathie, une ataxie cérébelleuse avec atrophie cérébelleuse à l’IRM et une atteinte vestibulaire. C’est une maladie lentement évolutive, d’étiologie encore inconnue, touchant l’adulte entre 50 et 60 ans. Typiquement l’IRM montre une atrophie du vermis antérieur et dorsal (lobes VI, VIIa, VIIb) et une atrophie hémisphérique crus I (correspondant au lobe VII du vermis). L’examen vestibulaire montre une anomalie du réflexe oculo-vestibulaire et une aréflexie bilatérale aux épreuves caloriques. L’électromyogramme révèle une atteinte sensitive de type axonale ; typique d’une ganglionopathie. Des critères diagnostiques ont été proposés en 2016 afin d’uniformiser le diagnostic du CANVAS. Nous rapportons le cas d’une femme de 78 ans présentant des troubles de l’équilibre en rapport avec un CANVAS, et décrivons les principales caractéristiques de ce syndrome.

CANVAS (cerebellar ataxia with neuropathy and vestibular areflexia syndrome) is a rare neurological syndrome including sensory neuronopathy, cerebellar ataxia with cerebellar atrophy on magnetic resonance imaging (MRI) and bilateral vestibular involvement. The etiology of this slowly progressive disease is undetermined. CAVAS affects adults aged 50–60 years. Typically MRI highlights anterior and dorsal vermis atrophy (vermal lobules VI, VIIa and VIIb) and lateral hemispheric atrophy affecting crus I (corresponding to vermal lobule VII). Vestibular examination shows abnormal vestibule-ocular reflex and bilateral vestibular areflexia. Electromyography reveals axonal sensory neuropathy; which is typical for a ganglionopathy. Some diagnostic criteria have been proposed in 2016 to standardize the diagnostic of CANVAS. We report the case of a 78-year-old woman presenting balance disorders related to CANVAS, and we describe the main characteristics of this syndrome.
