Nous rappelons dans cet exposé l'anatomie en IRM, les techniques particulières à l'étude de la région et les indications de cette méthode d'imagerie, ainsi que l'aspect des principales pathologies du temporal, en fonction de leur localisation. L'imagerie du temporal repose sur le scanner en haute résolution, qui sert en particulier à explorer l'oreille. L'IRM du temporal réalisée avec une antenne de surface ou une antenne tête est complémentaire de ce bilan en cas de lésions expansives tumorales ou infectieuses et permet certains diagnostics difficiles en scanner comme ceux de kyste épidermoïde, granulome à cholestérine, masse transtegmentale. Elle est indispensable pour analyser le paquet acoustico-facial, pour distinguer une cicatrice d'une récurrence cholestéatomateuse devant un comblement non spécifique de la cavité opératoire après traitement chirurgical d'un cholestéatome, pour confirmer le diagnostic de tumeur glomique tympanique, préciser l'état du nerf cochléaire, du labyrinthe et de l'encéphale avant la pose d'un implant cochléaire.

The purpose of this review article is to describe the normal anatomy, the specific imaging techniques and the applications of MR imaging to temporal bone pathologies. Though the evaluation of the ear is mainly based upon CT, MR imaging is essential in the work-up of tumors and inflammatory diseases, such as epidermoids, cholesterol granulomas, paragangliomas and transtegmental masses. MR Imaging is mandatory in the evaluation of the internal acoustic canal and the cerebellopontine angle. Finally, MR imaging plays an important role in the detection of recurrent cholesteatoma after middle ear surgery, and in the work-up of the labyrinthic cavities before cochlear implantation.
