L’oxygénothérapie nasale à haut débit est un dispositif connu en réanimation pour traiter les insuffisances respiratoires hypoxémiques modérées à sévères sans recourir à l’intubation. Ce dispositif peu invasif possède certaines caractéristiques intéressantes comme le lavage de l’espace mort respiratoire, l’inhalation de gaz humidifié et réchauffé ainsi qu’un contrôle sur la FiO2 permettant d’oxygéner un patient a FiO2 100 %. Il semble utile de proposer l’utilisation de ces dispositifs au bloc opératoire au cours de procédures effectuées sous sédation légère permettant de maintenir une oxygénation satisfaisante ou au cours de la laryngoscopie. En postopératoire, ce dispositif pourrait avoir un intérêt pour lutter contre l’hypoxémie secondaire à l’hypoventilation alvéolaire et au dérecrutement alvéolaire, tout en diminuant le travail respiratoire.

High-flow nasal oxygen therapy is a treatment preventing from intubation in acute respiratory failure associated with mild to moderate hypoxemia in ICU patients. It provides supplemental humidified oxygen with a good control of inspiratory oxygen fraction, a respiratory support with reduction of dead space and a dynamic positive airway pressure and improves the patient’ comfort. It could be used in the OR during procedures under intravenous sedation, and also to maintain an oxygen supply during the orotracheal intubation. Postoperatively, it could prevent hypoxemia related to alveolar collapse, and decrease work of breathing. However, it has no effect on post extubation hypoxaemia in cardiac of abdominal surgery compared to conventional oxygen therapy.
